While climbing off of the Mine Cat explosive carrier the miners tools became entangled on the tractor causing the miner to lose his balance and step down hard on the left leg. The miner reported hearing his knee pop and felt pain.
Employee was stepping off his drill and rolled his ankle. Seen at the ER and released to restricted duty for one day.
Employee was taping transducer wires to PVC under the table when he was struck in the mouth by a PVC lifter that was knocked off the table. Employee suffered three fractured teeth.
Employee was putting a screen cylinder on a storage rack and his left ring finger was caught between two cylinders that rolled into each other. Seen at the ER and stitches were required. Released to restricted duty.
Employee was loosening bolt with an impact gun. The bolt came loose, striking the employee above the left eye on the forehead. Seen at ER where stitches were required. Released to full duty.
Main door on bus was locked and employee tried to enter bus through the emergency door, which is approximately 3 feet off the ground. The employee slipped and cut his left index finger. He was seen at the ER and the laceration was stitched. Released to full duty.
Reported his right arm was sore. He did not recall anything happening to cause his arm to be sure. Through the night his arm sorer and was not getting better so he went to see doctor.
Employee was using a shovel to clean in the reclaim tunnels. The shovel handle got caught in the conveyor, pulling both hands into roller between belt. The employee had surgery for fractures in both thumbs. Also a fracture in right wrist.
Employee was cleaning dash of haul truck cab when he felt pain in his left lower back. It became worse and he was seen at the ER on 11/28 and released to full duty. On 11/30 he was seen again as the pain had increased, and he was put off work as site could no accommodate restrictions.
EE was changing cutting edges when one fell on his foot. He was seen at the ER and diagnosed with a fractured big toe. Released to restricted duty that the site was unable to accommodate. Lost time.
Employee was using an overhead crane and a chain hoist come along, and the chain hit him in the back of the hand. He was seen at the ER, put on restricted duty for two days, use of left hand only. Released to full duty effective 11/15/2011.
Helper was closing the rotation guard on the drill when his left finger tip was pinched between the frame
Tripping out rods when threads caught strap which wrapped around rod catching employee's hand between the rod and strap.
Employee walking on steep slope with uneven and floating rocks all day on the 26th. Her ankle began to hurt, the next day her entire right leg from ankle to low back was hurting. Diagnosed with sprain ankle and inflamed sciatic nerve. Restricted duty for 7 days.
While trying to remove a screw from the floorboard of a backhoe, the employee felt a pull to the right shoulder. Felt pain that worsened over the remainder of the shift, was seen at ER and orthopedist, diagnosed with a partial rotator cuff tear and sent to physical therapy.
Employee was opening a can of welding rod when the sharp edge of the pop top of the can cut his right palm at the base of the thumb. Stitches were required and the employee was placed on sedentary duty. Referred to an orthopedist.
Employee removing bolts from discharge pipe on barren pump. Nut came out of socket that was still in motion and struck him in top front 2 teeth and broke them. Saw dentist and will be having a root canal and caps placed on teeth. Employee released to full duty.
Employee was installing a fan extension and the guard slipped and cut the back of his left hand. Seen at the ER, stitches and put on light duty for 10 days. No light duty available, employee put off work for 10 calendar days, resulting in 5 lost work days.
IW stated that her arm was sore on 07/21/11. On 07/27/11 she was using tape making signs and her arm was sore again. Seen at ER on 07/27/11, diagnosed with strain and released to sedentary duty for one day. Investigated by workers' comp and site, determined work related on 08/15/11.
Employee was running out rods and slipped and fell dropping rod on foot.
Employee was checking to see if core was inside of core tube with his hands. While his hands were at the end of the core tube, the tube was being lowered at the same time and his fingers became pinched between the tube and the floor.
Employee put on fire jacket before loading furnace. When she took off jacket to go to break she felt burning and noticed a bite on her left arm. She was seen at the ER, diagnosed with an infected insect bite and prescribed antibiotics. Released to full duty.
IW was descending escape ladder in conduit, foot slipped off rung and he fell about 5 feet, landing on metal grating. Right ankle buckled under him. Xrays negative, placed in immobilizer boot. Released to full duty.
Employee helping to replace brake drum on shovel, rolled new drum into house, lifted it, and it tipped and smashed the tip of the right ring finger between drum and equipment. Received 7 stitches and released to full duty.
Employee was removing a pin on the rear axle of a piece of equipment when the pin came out and dropped on his left thumb. He was taken to the hospital where the thumb was surgically repaired and he was released to sedentary duty. He was subsequently placed off work.
While using a folding pocket knife on strap of vent back, knife slipped, cutting left index finger. Received 5 stitches and released to full duty.
Driller lifted the foot clamp (approx weight 120 lbs) off the drill table and twisted, causing back strain.
Walking out to service truck in ice and snow, slipped and fell, broke left leg and ankle.
Employee was assisting tripping out 5 1/2" rods when he felt a pop in his right calf.
Employee was tripping out a rod and felt a sharp pain in his left shoulder blade. The employee stopped, notified supervisor, stretched, applied ice/heat and then continued normal duties for two days before deciding to seek medical attention.
Ee getting loaded at shovel 352 when the bucket of 352 was dropped into the bed of the truck 303. This moved the employee upward and side to side within the cab. She was seen at the ER, diagnosed with a cervical and thoracic strain and placed on restricted duty which the site could not accommodate. Lost time as of 04/19/11.
IW operating dozer, felt pain in knee. No accident. Claim accepted as work related knee strain on 06/14/2011 after independent medical evaluation. IW had one day restricted duty on 04/14/2011 after receiving a steroid injection.
Individual was found unresponsive, pulse less, and breathless in wash bay. CPR was initiated on scene. Individual was transported to Northeastern Nevada Regional Hospital via Crescent Valley Ambulance. Initial determination is that this is a health related incident.
While tripping in 7 5/8 casing, employee pinched his fingers between the end of the sling device and the casing.
Employee had grabbed the casing with his fingers inside of the casing. The Driller opened the chuck, releasing the sub, causing severe lacerations of the employee's hand.
Employee was performing a retorque on the left rear of a Komatsu 930 with a RAD 30 gun and he smashed his finger. He was seen at the ER where xrays showed a fracture to the left 4th finger. 5 sutures required to close the laceration.
Employee was getting off bus to start night shift. He caught his left heel on the edge of the concrete pad, causing him to twist his left ankle. He was seen at the ER and subsequently released to restricted duty. Investigation complete on 01/18/2011, claim accepted by workers' comp on 01/25/2011.
While placing pipe on a scissor lift, pipe fell about 6 inches lacerating the employee's left pinky finger. Four stitches required to close wound.
While fixing liner in SAG mill, employee's right hand pinched between the liner handler and the SAG mill trunion. Xrays at ER showed right wrist fracture and employee referred to ortho. In a short cast and off work.
Emp bent over, opened a 45 pound metal cover for gas meter and felt pain in lower back upon pulling and turning to move cover. Investigation completed and determined work related on 12/21/2010.
Employee was removing a header and a high pressure hose hit him in the face. He received 10 stitiches to close a laceration on his right cheek. Released to full duty.
Emp climbing in and out of hopper chipping frozen mud. His right hip was hurting, but no accident. He saw doctor on 12/15 and on 12/20 investigation completed and claim accepted by workers' comp. Emp having electrical stim treatments at chiropractor, making this medical treatment.
Employee's hand slipped while putting inner tube back into the hole. This caused the employee to pinch his fingers between the rod and the latch head. The weight of the tube caused a laceration of the left ring finger and the left pinky finger of the employee.
Employee was washing under a conveyor belt with process water that has a high P.H. level. The injury arose from contact with the process water and a reaction between his skin and rubber gloves.
While changing hoist ropes on a shovel, ee right foot was sucked in between the equalizer and ropes. The tension on the rope got tighter, clamping the ee foot. Xrays were negative for fracture, diagnosed with a strain. Released to restricted duty, on crutches and boot. Placed off work.
Employee started experiencing pain in his wrists which was later diagnosed as bilateral tendonitis.
While unloading walkway extension the support leg folded up on the employee's left ring finger resulting in laceration and loss of fingernail.
Employee was lifting a box out of the back of his work truck, and felt a 'twinge' in the groin. He continued to work and at about 8 am he lifted another box and felt pain. He was seen at the ER, diagnosed with a hernia and referred to a surgeon. He will have hernia repair surgery on 11/02/2010.
Lifting casing with jib boom when casing became hooked on threads of other casing allowing C hook to come out and casing to fall back to truck hitting employee's foot
Driver was climbing up onto work truck to close bay door, stepped off and landed the wrong way causing ankle to break.
