While climbing off of the Mine Cat explosive carrier the miners tools became entangled on the tractor causing the miner to lose his balance and step down hard on the left leg. The miner reported hearing his knee pop and felt pain.

Employee was putting a screen cylinder on a storage rack and his left ring finger was caught between two cylinders that rolled into each other. Seen at the ER and stitches were required. Released to restricted duty.

Employee was loosening bolt with an impact gun. The bolt came loose, striking the employee above the left eye on the forehead. Seen at ER where stitches were required. Released to full duty.

Main door on bus was locked and employee tried to enter bus through the emergency door, which is approximately 3 feet off the ground. The employee slipped and cut his left index finger. He was seen at the ER and the laceration was stitched. Released to full duty.

Reported his right arm was sore. He did not recall anything happening to cause his arm to be sure. Through the night his arm sorer and was not getting better so he went to see doctor.

Employee was using a shovel to clean in the reclaim tunnels. The shovel handle got caught in the conveyor, pulling both hands into roller between belt. The employee had surgery for fractures in both thumbs. Also a fracture in right wrist.

Employee was cleaning dash of haul truck cab when he felt pain in his left lower back. It became worse and he was seen at the ER on 11/28 and released to full duty. On 11/30 he was seen again as the pain had increased, and he was put off work as site could no accommodate restrictions.

Employee walking on steep slope with uneven and floating rocks all day on the 26th. Her ankle began to hurt, the next day her entire right leg from ankle to low back was hurting. Diagnosed with sprain ankle and inflamed sciatic nerve. Restricted duty for 7 days.

While trying to remove a screw from the floorboard of a backhoe, the employee felt a pull to the right shoulder. Felt pain that worsened over the remainder of the shift, was seen at ER and orthopedist, diagnosed with a partial rotator cuff tear and sent to physical therapy.

Employee removing bolts from discharge pipe on barren pump. Nut came out of socket that was still in motion and struck him in top front 2 teeth and broke them. Saw dentist and will be having a root canal and caps placed on teeth. Employee released to full duty.

Employee was installing a fan extension and the guard slipped and cut the back of his left hand. Seen at the ER, stitches and put on light duty for 10 days. No light duty available, employee put off work for 10 calendar days, resulting in 5 lost work days.

Employee was checking to see if core was inside of core tube with his hands. While his hands were at the end of the core tube, the tube was being lowered at the same time and his fingers became pinched between the tube and the floor.

Employee put on fire jacket before loading furnace. When she took off jacket to go to break she felt burning and noticed a bite on her left arm. She was seen at the ER, diagnosed with an infected insect bite and prescribed antibiotics. Released to full duty.

IW was descending escape ladder in conduit, foot slipped off rung and he fell about 5 feet, landing on metal grating. Right ankle buckled under him. Xrays negative, placed in immobilizer boot. Released to full duty.

Employee was removing a pin on the rear axle of a piece of equipment when the pin came out and dropped on his left thumb. He was taken to the hospital where the thumb was surgically repaired and he was released to sedentary duty. He was subsequently placed off work.

Ee getting loaded at shovel 352 when the bucket of 352 was dropped into the bed of the truck 303. This moved the employee upward and side to side within the cab. She was seen at the ER, diagnosed with a cervical and thoracic strain and placed on restricted duty which the site could not accommodate. Lost time as of 04/19/11.

Individual was found unresponsive, pulse less, and breathless in wash bay. CPR was initiated on scene. Individual was transported to Northeastern Nevada Regional Hospital via Crescent Valley Ambulance. Initial determination is that this is a health related incident.

Employee's hand slipped while putting inner tube back into the hole. This caused the employee to pinch his fingers between the rod and the latch head. The weight of the tube caused a laceration of the left ring finger and the left pinky finger of the employee.

While changing hoist ropes on a shovel, ee right foot was sucked in between the equalizer and ropes. The tension on the rope got tighter, clamping the ee foot. Xrays were negative for fracture, diagnosed with a strain. Released to restricted duty, on crutches and boot. Placed off work.
