Roof fall dated 6/3/2014 was located in the #6 entry of the mains in by spad #530. The fall was approximately 40'L x 18'W x 10'H.

Employee was walking toward the outside in the secondary escapeway, and slipped injuring his left calf.

IE was roof bolting, started to insert his pusher steel when a rock fell and struck him on the left forearm causing a laceration.
Employee was operating the #2 Fletcher Roof Bolter in the #4 entry of MMU, 001-0. As employee was drilling his wrench kicked out striking his little finger on his left hand causing a dislocation.
Employee was operating the scoop in the #9 entry. As he removed himself from the scoop, to retrieve some rock dust he slipped, and landed on his left hand, causing a sprain to his wrist.
Employee was inserting roof bolt into hole. The bolt bent, and the ring finger on his left hand was smashed against the mast on the Roof Bolter which, resulted in employee receiving two stitches in his finger.
EE was building a intake stopping, as he placing the block, the ladder started to shift causing ee to fall. As ee was falling the block fell striking him on his left collar bone causing a fracture.
A piece, of rock fell across the miner. While, ee was removing the rock with a slate bar, the rock broke in half, and the slate bar slipped striking ee in the nose causing a laceration. After finishing the shift ee went to the Doctor, and received butterfly stitches.
Employee was standing on a ladder installing fly pads in the #2 entry. While hanging the fly pads, the scoop operator trammed through pads striking the ladder, causing him to fall.
EE was installing drill wrench on his steel, the wrench slipped off striking his left hand, causing his thumb to be dislocated.
EE was pulling rubber coating off of shuttle car cable. While ee was pulling his channel locks slipped striking himself in the mouth.
EE, was attempting to remove a piece of loose rock, from around and roof bolt. The rock fell out hitting EE, on his left hand causing 1 inch laceration, on his left hand.
Employee was roof bolting on MMU 004-0 in the #4 entry. While putting up his inside bolt a piece of rock fell measuring 39" x 26" x 6" came inside the man-up striking employee in his left leg, causing tear to the Achilles Tendon.
Employee was standing in the #3 entry when the shuttle car started up the cable, struck him in the back causing him to fall on his back.
Employee was installing rib straps on MMU 004-0. While making a sudden turn, he felt a burning sensation in his lower back on 4/20/12. On 6/8/12 his back was hurting so he went home early. That afternoon, he went to visit his Doctor concerning his back and is off by Doctor's orders until further notice.
Roof Bolter operator was bolting top in the #1,entry while removing his drill steel a piece of rock fell measuring 17"x13"x4" thick. The rock struck the victim on the left wrist causing a laceration.
Loading belt structure in intake in insert in the scoop for next belt move and top roller struck small finger on the right hand and busted finger open. Received stitches in finger.
Working on the starter box and while standing on a rubber mat, when turning his feet slipped on the wet mat and he pulled a muscle in the back of his left leg.

While pulling rock off of the Continuous Miner with the scoop, the rock broke and landed in the scoop bucket bouncing the scoop operator (victim) against the roof.
While installing roof bolts, he stepped forward to insert glue in the hole and twisted his knee.
Victim was cutting a clip off the belt when he slipped and the knife cut his hand.

Victim hit his left hand with a hammer. His safety glasses were fogged up
Employees were re-setting timbers in the intake. While attempting to set a 14' timber, they lost control and the timber fell over and struck the victim on the back and pushed him to the ground.
Holding a board over a kettle bottom for the other bolt operator to bolt up. When the stab jack on the machine was set, it pushed the board against the roof pinching the victims finger
Victim was cleaning rocks and coal to set timbers along a rib line. The rib and brow fell crushing the miner. The brow measured approximately 100" long by 32" thick, by 37" tall.
