Retrieving core samples from core barrel, sample was lodged. Employee tapped on barrel with hammer to dislodge the core. When core dropped, it was jagged on end and it slashed into driller's hand.

Employee was trying to repair lights on top of skid steer. Employee was standing on the entrance exit platform attempting to bend the cab shield up to repair lights. Wrench slipped causing employee to fall onto the bucket dislocating right shoulder.

Ind. Hauler was attempting to leave the quarry site when he lost control of his truck causing it to overturn. The scale house clerk stated that the truck driver was talking on his mobile phone has he left the scale. The driver was distracted and works for Kentucky Concrete.

EMPLOYEE CLIMBED UP ON LOADER TO REPLACE WIPER ASSEMBLY. EMPLOYEE HAD WINDSHIELD WIPER ASSEMBLY IN HIS HAND. EMPLOYEE STEPPED OVER ON FRONT FENDER AND FOOT SLIPPED OFF FENDER STEP AND EMPLOYEE FELL AND FRACTURED HIS RIGHT HEEL.

Employee was mounting a cylinder on a haul truck and caught his finger between the cylinder and the tank. Employee pulled the cylinder and jerked his hand out causing injury to the left index finger. Employee was transported to a local Dr. Michael Carter. The employee received stitches, a mandatory company drug test and returned to work approximately 1.5 hours later.
