Employee entering lab office from shaker/oven room when office door hit her heel as it closed. Laceration was bandaged and she remained at work. That night, employee concerned when changing bandage. Called employer who told her to use her judgment regarding seeing doctor. Went to ER and received 8-9 stitches. Employee at work on next scheduled workday (8/23).

Showing all 3He slipped on muddy track while mounting dozer, fell to track & hit L knee on blade rail & stomach on the track. Reported fall the next day, saying he was just sore. We sent him to dr on 1/27/10 after he complained of stomach discomfort & swelling knee. They performed x ray of knee and ct scan of stomach. set up an appt. for him to see a surgeon on 2/2/10 about stomach hernia.
Two employees cleaning out a plugged vibrating screen discharge chute - one at top and one at bottom. Rock dislodged and fell down chute, hitting employee's finger. Employee not wearing gloves. Cut end of finger. X-ray showed slight fracture of finger tip. Six stitches required. Employee returned to work next day.
