While loading bolter supplies onto scoop bucket the employee twisted his back.
A roof fall was found in 9 mains-#5 Butt-#3 Entry approx 40' inby station #5190. The fall was approx 6' high, 19' wide and 40' long. The area was bolted with 6' fully grouted torque tensioned bolts. The area is dangered off. The fall does not impede ventilation or travel.
The employee slipped and fell backwards when pulling a discharge line injuring his back. He continued working until 6/1/09 when he had surgery.
The employee was operating a shuttle car down grade toward the feeder when a piece of rib (4.5'L x 2'W x 0"-12"T with a 2" rock binder) fell on his leg. The employee reported the piece became wedged between the rib and his chest before he got the machine stopped. He received bruised across his chest and legs.
