No curbs on the exit lane to align tractor-trailers to the overhead safety cage. Driver pulled under the safety cage at a slight angle and the safety cage would not lower all the way down onto the tanker because of the adjustment chains. Driver stepped onto the top of the tanker and lost footing and fell between the safety cage and the tanker landing on the ground below.
Maintenance Mechanic was pulling on gearbox during removal and shortly after noticed a bulge in their stomach area.
Employee states EE tripped over fire extinguisher & fell onto a pallet that was on the ground, fell onto EE's side.

Fork Lift operator lifted an inner drum using a cable for securement. Employee tried to shift the drum on the forks from the side when it fell off and landed on right foot.
Employee felt ill - like heart attack, called 911 for transport to ER - initial from EMT is most likely a panic/anxiety attack.
Miner entered the Kiln to perform an inspection. Made their way to the kiln inlet to take a look at the sugar scoops and was taking pictures of the brick that did not get removed. In the process of taking pictures, lifted up their head and a small clump of kiln dust fell from the bricks and landed between hard hat and safety glasses eventually moving into right eye.
Employee was holding a portable 2-way radio microphone in right hand. Another employee pulled it away and the metal bracket on the back of the microphone cut the right middle finger with a deep gash. EE was sent to ER and received 3 stitches to seal the wound.
Employee was walking from office to the parking area - one part of concrete slab is raised appx. 1 inch across . Employee tripped on the raised edge and fell to the ground causing a left small finger middle phalanx fracture
Walking on the concrete to get a loader, slick from mud and rain, employee slipped but did not fall. Strained lumbar region.
Was removing a plate from the west drag when it slipped from a pry bar that was holding it up and pinched the employees left index finger against a washer of the long bolt EE was taking out, laceration on the front side first and second sections - 4 sutures and splint.
Employee was using a grab bar to pull self up into the cab of a grader when EE felt a tug on the shoulder - after consultation EE may have a bone sour/bursa sac inflammation or an impingement - MRI may be needed to decide.

Worked Full Duty till LDW 8/5/19 - Loading a shot on uneven ground, as EE turned from one hole to the next EE lost footing on a rock, hurt right knee.
Employee plus other maintenance and electrical employees were changing out the slide on the #33 silo. The slide slipped off and landed on the employees left foot just above the steel toe protection. We immediately took the employee to a local ER for X-Rays and confirmed that foot was broke. EE was place on light duty and referred to TOC

Employee requested and received follow-up treatment on 5/2/2017 due to continued pain and was diagnosed with lower lumbar strain and prescribed physical therapy. Incident became reportable on 5/2/2017 due to being prescribed PT.
Employee was assisting in manually moving a plate of steel and dropped it resulting in right, middle finger being caught under. The injury resulted in a laceration and fracture to the finger.
Employee was assisting other employees in placing a shaft into a mechanical separator. The shaft shifted and caught the employee's right thumb between the shaft and separator wall. The incident resulted in a laceration to the right thumb.
Chain fall was hooked to liner that had pad eye welded to it. The pad eye broke loose and liner fell and hit EE in the right thigh. EE received a laceration on right thigh.
Employee was walking across the maintenance shop when EE was struck by a particle ejected by a nearby task (portable grinder). Foreign object in left eye. Object was not imbedded and was flushed out. Required Rx medication for eye.
Employee had some cement dust get into left boot causing a cement burn to the outside of left ankle. Medical treatment was given and placed on restricted duty. The employee reported the incident to supervisor on 9/17/2016.
