Plastic water line was frozen. EE was using a propane weed burner and air to move the ice. The ice broke free and several small chunks of ice struck EE in the left brow, causing a laceration, resulting in 3 stitches.

Miner was drilling with a jackleg drill when a rock fell 8' from the back and struck the miner on his left arm. Miner received 12 stitches to his arm.

Employee was picking up a jackleg drill after finishing drilling a knee hole when he felt a very sharp pain in his lower back.

Employee was holding onto a 2" air hose, and his co-worker opened the air valve to clean out the air line. The resulting pressure was more than employee could handle and the hose struck him in the left knee.

The EE shanked a striker bar took striker bar off of the longhole drill machine and the 30 feet of drill rod fell out of the hole and struck the EE on the left hand. Required 5 stitches.

The employee was backing up a Wagner 6 ton haul truck the ee had a 6 foot drill steel in the operator station of the truck. This steel was caught on the (rib) of the drift and hit the ee in the head behind the left ear.

EE was drilling a face round with a Gardner Denver Jackleg the miner's foot was between two rocks on floor of heading the weight of the drill and steel coming out of the drillhole caused him to turn his body and his foot between the two rocks,his foot would not turn and this caused a break in a bone in his left foot.

The EE was working on a 480 electrical eq. disconnect, changing cord end for Jumbo. The electric power was not turned off and locked out. The EE made contact with the 480 volt circuit and fell to the floor of the drift. EE could not let go of the circuit and EE's in the area pulled the cable out of his hands.

The miner was climbing up the charley south raise and a small loose rock fell down the raise and striking the miner in the face on the chin and breaking a tooth and neck pain. Also visual discomfort to the right eye.

The EE was installing split set roof bolts with a jack leg drill. While driving the bolt into place the steel plate on the roof bolt knocked a rock out of the back. The rock hit the miner in the head, his hard hat was on his head pushing his face onto the jackleg drill, breaking his tooth.

The miner was working in the Charlie south raise he was lifting a large timber 8" X 8" X 10' and pulled the muscles and tendons around his left elbow. The accident was not reported until 8/19/10. The employee said that his arm hurting, a lot of pain and needed to go to hospital to have x-ray.

The miner had just ate lunch, walking back to work area, a rock fell out of the back and struck the EE in the head. The EE was taken to the hospital on 8/15/10. The accident was not reported until 8/15/10 because the miner was observed underground and on 8/15/10 and did not know who he was. Was told at the hospital by his wife about the rock striking him.

The miner had turn the air off to the jackleg drill to put oil in the oiler, The jackleg was against the rid of the drift. The miner turn the air on and did not jackleg drill went up rib and falling onto the miner right leg. Bruised right knee with swelling.

The EE (miner) was loading prill into face round with a prill pot. Walking toward the prill pot and weld on top of prill pot came loose and blew prill into his face, a lot of prill.
