Showing all 4Employee was changing a fuel filter on a pump. Employee grabbed the fuel filter with his right hand and the fuel line with his left hand to remove the fuel filter. While twisting and pulling the fuel filter the fuel line released suddenly and his left hand hit the frame of the pump. Employee's left third finger was cut on the outside of the first knuckle and required five stitches.
Employee was walking to water treatment pond after it rained when he slipped and fell in the mud, twisting his ankle. This caused his left ankle to be dislocated and left leg to break.
Employee slipped and twisted back while going over berm to start and fuel pump. Heavy rains overnight.
