Employee was helping move roof bolter when the operator started the roof bolter EE turned on the cable reel it caught the employee's feet causing EE to fall down. As EE was falling, EE tried to catch self with EE's right arm causing pain in EE's right arm and shoulder.

On 8/28/2019 a Non-Injury Roof Fall occurred at this mine. The roof fall is located in the No. 6 entry at x-cut No. 24 on the 1-South Mains. The MSHA Hotline was notified and MSHA has conducted an investigation of the Roof Fall. The Roof Fall is approximately 18' feet (There are no injuries associated with this Roof Fall).

Employee was loading roof support material on the roof bolter. EE bent over to pick up roof bolts and a small piece of rock fell from the roof and struck EE in the back of head causing a small cut to the left side of the head.

A non-injury roof fall was found in the No. 6 intersection and the No. 6 right crosscut at crosscut No. 7 on the MMU No. 002-0 section on the 1-South Mains. The roof fall is approximately 20 feet wide x 35 feet long x 12-16 feet high. The roof fall will be cleaned up and the mine roof re-supported. The MSHA Hot-line was notified of the roof fall. See map. There were no injuries.

The injured employee and co-working were removing a metal canopy from a roof bolting machine. As the employees began to lift the canopy with a chain and ratchet to re-position the canopy, the chain and canopy slipped, allowing the canopy to shift, which resulted in an injury to four fingers on the left hand. (No broken bones). Cut glued together.

The employee was plastering a concrete brattice. While moving the bucket of plaster, the employee dropped the bucket and the plaster splashed into the employee's right eye. The employee worked the remainder of shift. On 11/20/2018 the employee's right eye was red and slightly swelled. The employee received medical treatment for the eye injury on 11/20/2018.

The employee was walking down three (3) steps to the concrete basement floor. As the employee was stepping from the last of the 3 steps to the basement, EE caught heel on the last step or the left foot slipped and EE tried to catch self to keep from falling which resulted in an injury to the left ankle.

An unintentional non-injury roof fall was discovered while doing weekly exam, in the #1 entry of the #1 east mains return, approximately 130 feet in-by the mine portal. The fall was reported to MSHA Hotline, the fall will be cleaned up and re-supported. The fall is 16' wide and 4' high and 4' to 5' long.

The employee was in the belt entry using a roof bolting machine to install belt hangers. As the employee was installing the belt hanger ee reached to activate the fast feed lever when ee twisted lower back.

The (contractor) roof bolter employee was in the No. 1 entry. The roof bolter operator started to drill the hole and a piece of draw rock fell out mashing the contractor employee's ring finger and third finger on right hand against the drill pot of the roof bolting machine. This resulted in a break and laceration to the ring finger on the right hand.

The roof fall is approximately 28' x 30' ft long x 18' x 20' ft wide x 10' x 12' ft thick. The roof fall will be cleaned up and the mine roof re-supported.

EE was using a large hammer to hit on a belt roller to break it loose from the steel structure. EE was holding the roller with their left hand. As EE was swinging the hammer with the right hand and hitting the roller, they missed the belt roller and hit a finger with the hammer. This resulted in a laceration to the left ring finger, requiring six (6) sutures to close the wound.

A non injury roof fall occurred at approx. 9:25pm on 11/3/17. The roof fall is located in the No.4 entry intersection at x-cut No.8 on the 1 east mains. The roof fall is approx. 20' feet long x 20' feet wide x 8' feet to 10' feet thick. The roof fall is confined to the intersection at x-cut No.8 The area has been flagged and dangered off to prevent access to this area.

Non-injury roof fall was found in 5 left panel off 4-North Mains. The roof fall was found during a weekly examination. Roof fall begins in No. 5 entry of 5 left panel and extends inby to crosscut No. 12. Roof fall then continued into No. 6 entry of 5 left panel. Roof fall appears to be approx. 4ft to 8 ft. thick and approx. 18 to 20 feet wide (see map). MSHA Hotline notified.

Employee was raising the boom on the belt-head drive. EE was twisting screw jack on the belt drive when EE slipped and fell twisting EE's right knee. The accident occurred on 8/22/17. The EE returned to regular duties after the accident and continued to work until EE's shift on 8/25/17 - 8/26/17.

The employee was walking beside the Rail Road cars at the load out facility checking the doors on the cars. The employee stepped onto a rock and the rock rolled over and the employee fell into a small ditch. The employee continued to work and did not receive any medical treatment until 7/19/2017.

The fall has been dangered off/wooded cribs installed on all sides of roof fall.

Contractor EE & co-worker were on the surface working on a rock dusting machine. After the repairs were completed, the IE was standing in front of the machine when the operator started the machine for testing. The IE started to step back & lost balance. IE reached out to grab something & got finger into an unguarded location resulting in cut to tip of finger.

The roof bolter operator was loading metal straps onto the roof bolting machine. As the employee was loading the metal straps their hand started to slip and the metal straps shifted or slid and the corner of the metal strap cut the employee on the inside of the right forearm.

The injured employee was on EE's knees hanging the roof bolter cable slack. After the cable was hung, the operator started to get onto the roof bolter. The operators hand slipped and EE pushed down on the automatic cable reel take up. The injured EE was close to the cable and when the reel engaged the cable became tight and struck EE pushing EE to the side, twisting EE's left knee.

Roof fall occurred in the No.3 entry roadway at crosscut No.26 of the mains. Roof fall is approx 35'-40'long x 30'wide x 6'-10'thick. Roof fall was reported to the MSHA hotline. Call confirmation number 00016188. Wooden cribs have been installed on inby side of the fall and in the adjacent cross cuts to the fall. Mine was in a non-producing status at the time of the roof-fall.

The injured employee was attempting to remove a cord lash or nylon band from a bundle of roof bolts. The employee was using a utility or razor knife to cut the band on the bundle of bolts. As the employee used left hand to cut the banding material, the knife slipped and cut right index finger.

The employee was using a rock bar to pry on a bundle of metal straps to get to a bolt on the back of the roof bolting machine, when the bar slipped causing the employees right arm to come in contact with the corner of the metal straps resulting in a laceration to the right wrist when required (5) stitches or sutures.

Contractor employee was performing service work on a continuous mining machine. The contractor was attempting to unpin the short side of the rub rail to clean the demister on the machine. When the rub rail was unpinned both sides of the rub rail came out at the same time. The contractor put the rub rails back into place and strained his back.

The injured employee and Co-worker was removing the slack from the conveyor chain of a continuous miner. The employee had removed links from the conveyor chain and was re-assembling the pins and keepers in the chain. A sledge hammer was being used to drive the lock rings onto the pins when the co-worker hit the injured employee's left little finger with the sledge hammer.

The injured employee and co-workers were removing wooden timbers from the bucket of a scoop. The employee was down on one knee pulling the wooden timber out of the bucket when he felt a sharp pain in the lower right side of his back and right hip.

A no injury roof fall occurred at the no. 1 mine portal. The roof fall damaged the mine canopy at the entrance to the no. 1 entry. The roof fall was rock from the high wall and the edge of the mine roof for approximately 4'-6' feet underground. Upon discovery of the roof fall, MSHA was notified. The area was cleaned up and a new canopy was installed in the no. 1 portal.

The roof fall did not affect the mine ventilation or impede passage. The roof fall is located in the outby area of the mine. There are no injuries associated with this roof fall.
