The miner lifted a piece of floor grating, tilting it to the side against tunnel wall and then shoveled material that was being accumulated underneath. The miner turned around briefly when the unsecured grating slid and impacted EE's left hand/wrist.
Employee walking through the access below the Roman tripped and fell on their side injuring the right side of their chest.

Employee was repairing a battery of a Loader Caterpillar 980C, he turned his back to look for a grinder when the battery exploded. As a result of the battery explosion the battery acid came in contact with employee's left eye.
