At approximately 12:35 PM, a fire occurred at the TR Bolinder Crusher. Prior to the incident, maintenance activities were being performed on the guarding for the cone feed, during which a torch was used to clear bolt holes for installation of the guard. Upon completion of the task, three bolinder employees left the work area and returned to their trailer for a scheduled break.

Had just finished up workplace exam and shimming the cone, done pre-shift inspection and had blown horn for start up and around 8:19 am. Co-worker saw EE lying on conveyor belt with his legs under the magnet. He immediately notified the crusher operator and he immediately shut off the operation.
