The employee responded to a high-temperature alarm on the impactor crushers displayed on the HMI. As part of normal correction, the employee proceeded to the impactor building to activate the air horns for each impactor unit. The employee then proceeded to BIC2 and opened the air horn valve. A loose hose end became pressurized and struck the employee in the left eye.

EE was lifting a full oxygen bottle into a UTV and during this task the bottle slipped pinching their right ring finger in between the bottle and tailgate causing a laceration and fracture of the finger.

At approximately 10:55AM two employees reported to their immediate supervisor that they were stuck in the K3 elevator between the 5th & 6th floors. Executive Elevator was notified at 11:08AM and arrived onsite at 1:00PM. MSHA notified at 11:25AM (ref # 04688756). At approx. 1:28PM employees were released from the K3 elevator. MSHA was contacted again at 1:30PM (ref # 04689063).

EE was attempting to clear a dust collector vent line and in the process of doing so they got a fragment of material in eye. Although they were wearing safety glasses the material got into left eye. The material was removed at the clinic and the EE was given an eye patch and placed on restricted duty for 2 days.

EE was attempting to close passenger elevator door, in the process of doing so EE smashed middle finger. Injury was treated as a first aid. On 03/10/25 EE requested medical evaluation which resulted in EE's duties being restricted. EE was also given splint to immobilize finger.

Contractor was taking the elevator down to the ground with an oversized ladder and it shifted in the elevator causing the elevator car to rock which locked the governor. This is a safety feature of the elevator car.

EE was working as a tool handler when EE felt dust in eyes. EE visited a clinic on the same day to receive treatment. The doctor confirmed that it was dust that had caused the irritation in the eye. The doctor washed the eye and prescribed some eye drops. As per doctor's order EE resumed work after the treatment on the same day.

the laborer was tasked to unclog the ¾' Tunnel bunker so that material could flow through since it was plugged with wet material. The laborer stood outside the bunker and used an air-lance to dislodge material from below, material hit the air-lance and trapped the laborer's right forearm between the bunker's bottom opening wall and air-lance, causing muscle sprain.

elevator unexpectedly stopped between the 3rd and 4th floor of k2 tower. Several attempts to get the elevator unstuck were made and elevator maintenance contractor was called. The elevator stopped due to dust build up on the track of the elevator causing the governor to engage stopping the elevator. The track was cleaned and r inspection before putting back into service.

The step switch oil tank for raw mill caught fire likely due to the motor continuing to run after the command from the PLC was given to stop. The wound rotor leads flowed current to the high resistance contacts of the step switch which would generate considerable heat and eventually cause the oil in the tank to ignite.

EE was removing plywood form material with a pry bar. When pulling down to separate the materials, the bar slipped pinching the tip of the EE's right ring causing a laceration.

An EE was standing on a step ladder using a hammer to unchoke an air slide at the Plant. For unknown reasons they slumped over and slid down the ladder landing on their left side. EE was non responsive and workers provided first aid including use of an AED and CPR until EMS arrived and took over. EE was transported to the hospital, did not recover consciousness before they died.

EE was attempting to open a clinker rail car hopper door to dump the railcar using a prybar. The EE put their leg under the hopper door and when the door opened the clinker flushed onto EE's leg causing a burn to their right leg. The EE was taken for medical evaluation and placed on modified duty. The burn was cleaned and bandaged.

EE was walking down steps to access mens shower room when EE lost balance. EE landed flat footed on right foot and felt a sharp pain. EE had fractured the same foot 4 months prior. X-ray on 9-30-22 showed foot had been fractured once again in same area. EE was given restrictions and walking boot.

On 9/9/22 at 8:20am, employee was cleaning the walkway for C-16B because rocks had piled up after a spill. Employee was shoveling walkway and planted their left foot, pushing up against a guard with right leg to move it inward. Employee felt a shooting sensation of pain on right side lower back, groin, and down the leg.

The water truck tail light needed to be changed. The mechanic performed a JHA and was wearing rubber/latex gloves to handle the lightbulb of the tail light. As the mechanic went to turn the bulb to remove it off the truck, the bulb exploded, causing a laceration to the thumb and index finger of the mechanic's left hand. The employee was escorted to the nearest hospital.

EE removed the bolts on the torque arm for the airlock which was believed to be plugged. When the last bolt was removed the torque arm moved pinning the impact gun the EE was holding against the wall which was next to the airlock. This caused the EEs hand to be wedged between the wall and the impact gun causing a sprain to the wrist. EE was given modified duty for 5 working days.

Train mechanic was using a sledge hammer on locomotive when they hit themselves in the lip with the hammer, causing a laceration requiring one stitch to close. Employee was given modified duty.

Miner was operating CAT 326 long reach excavator in approx. 1ft to 3ft of water when left front track dipped and slid into aprox 15 - 20ft of water, the miner was able to escape the cab through the drivers door window and get to the surface. MSHA was notified and came to location same day. No injuries to the miner.
