In verifying operation of air actuated valve, employee placed hand on or near the valve seat and valve closed, pinching the middle finger of the right hand. Doctor diagnosed an open wound of right middle finger with damage to nail. Treatment included subcutaneous tissue repaired with 2 sutures and an injection of Ketorolac. Employee discharged as fit for duty without restrictions.

Showing all 12A temporary staffing service employee was washing a 250ml Erlenmeyer flask with a bottle brush. During this process the flask broke and a piece of glass penetrated a nitrile glove and cut the palm of the injured employee's right hand. The injury resulted in the employee receiving 6ea. stitches and a return to work release with no work restrictions.
100 lb. barite sack rolled onto shoulder of injured temporary staffing employee. Diagnosis of other sprain of left shoulder joint - minor and Pain in left shoulder - minor. Doctor released with no restrictions on 12/11/15. Sore shoulder prompts doctor's visit on 12/17/15 resulting in "Medium Work" restriction and left shoulder/scapular contusion diagnosis.
Employee was cleaning the plant area, rolled a 2" x ~15' suction hose, and placed on the floor. At the time the hose was placed on the floor he apparently twisted his back. An 11/9/15 physician's report indicates that the employee's diagnosis was "Lumbosacral strain / R Sciatic neuralgia", and states that the employee "is totally incapacitated at this time".
As the employee attempted to remove cattle from the mill site, a cow "charged" the employee. The employee jumped off an embankment to avoid the cow and injured his heel as he landed on the roadway. Accident did not involve compliance issues, mine equipment or mining system, or miner related skills training or attitude. Required PPE was utilized at the time of injury.
EE was lifting a flange onto a wheel when he felt pain in his lower right back.
When a fitting came loose with a wrench over it, it overextended the employee's arm pulling and stretching his arm, ribs, elbow and back.
Worker was working by air compressor fixing jig screens. The extra jig screens were resting against compressor. The compressor fell over, breaking one of the fittings. It caused a blast of air, the worker ended up on the floor-five feet from original position, and a cloud of dust occurred. the worker got metal fragments in his eye. He had his safety glasses on.
He was using the cutting torch and some of the dirt flew under my safety glasses and into his eye. Dirt in Right eye.

EE was taking trash from mill to dumpster outside of property fence using the forklift while dumping the trash some fell out. He parked and dismounted the forklift and was putting trash into the dumpster when he slipped in the mud and fell to the ground impacting his ribs on the top of a fork. The doctor cleared him for work on 5-23 which was his next scheduled shift.
Employee was shoveling mud (barite) up off the floor into the shaker (dewatering screen), which is about 5 foot high and his knee gave out on him. This is a repetition job to keep his area clear of hazards and clean as needed.
