EE was fueling an excavator using a tractor w/a fuel cell on it. When done the operator started the excavator and began to spin around and hit the fuel cell on the tractor. EE said EE's back hurt a little so I asked if EE needed a Dr EE said no and finished out the day. Mon EE left 12:24 Dr. apt. Tues 10/31 EE had a follow up will return to work on 11/3

On July 02 2012 employee was climbing up rear steps on a front end loader and slipped and hit his shin. He said he did not need medical treatment. On July 12 2012 employee asked to go to doctor for same injury.
