Showing all 23Injured employee had finished servicing the Cat 345LL track hoe. As he stepped down to dismount he felt a pain in his left leg & groin area. he tried to continue to work but as the night went on it got worst.
EE was watching the electric cable on a 2800 electric shovel which was moving. The cable had slack in it and as that slack was pulled out the EE was not far enough away from the cable when the slack came out striking him on his right knee dislocating it. As cable was pulled, the shovel door opened striking the cable jerking it into the EE.
Fuel Truck # K9079 caught on fire on top of hill at the Halls Creek mine on 2nd shift, operator was o.k. and no one was injured. Calls were made to MSHA and State Agencies informing them of the situation. (See Attached Sheet)
Injured party was pressure washing a hyd. cylinder with a pressure washer. He was using a 2' wand to wash with instead of the 4' wand. The wand slipped out of his left hand & caught his finger. He required 4 sutures to close up the wound. He came back to work & did not miss any work days.
Employee climbed down on a step ladder and when he got to the last step he stepped down onto uneven ground under the last step and hyper extended his right knee. Tore a tendon behind the knee cap.
Employee had just finished his shift and had parked the cat scraper he was operating. As he stepped down off the last step his foot got hung up in the last strap causing him to stagger back twisting his right knee.
Party climbed out of a vehicle & as he started walking he stepped on some frozen ground that gave way with him causing him to roll his right ankle. It was a severe sprain.
Party had got of his truck at the start of the shift to put a 6" pump line back together. It was down in the twenties temperature wise over night. The water on the ground froze. As he went to put it back together he slipped and fell injuring his toe.
Party was climbing down his haul truck. When he got to the last step to step down, his foot got caught causing him to fall backward on the ground with his foot still caught.
A Cat D11R Bulldozer was found on fire after work hours by an EE who was coming to work on the next shift. 2nd shift had parked the Cat D11R Bulldozer at 2:00AM at the end of the shift in the parking lot. EE coming to work on day shift at 3:30AM came upon the fire as he came in to work. No one was injured. Cat D11R Dozer was totaled.
Ray Jones Trucking who is a contractor CDA was dumping a load of coal from one of their coal trucks on the coal yard when the truck bed started leaning over and fell over on its side. No one was injured.
Party was climbing down off dozer to check something, when he stepped down his foot slipped and he fell, holding onto the dozer with his left arm.
Party was standing on the racks of a 475 Komatsu bull dozer replacing cylinder blade when he slips and fell off the dozer striking his right lower leg against the dozer requiring 11 stitches.
Party had just put water in water truck and was walking to get on water truck when he turned and heard his right knee pop. Started feeling pain soon after this.
Party was climbing the Cat 657E scraper on the rear. He was in the process of starting the rear engine when he fell off the Cat 657E scraper.
Injured party was disconnecting a pull rope off a piece of eq. He bent over and was pushing and pulling on the pull rope when he felt a pop in his chest.
Party lifted a sprocket segment which weights about 70lbs out of a box. He felt a pain in his lower back. Missed 1 day of work.
Party was doing a pre-op check on his Cat 777F back dump truck. As he grabbed a hold of a handrail to bend over and look at his truck engine a piece of rust fell behind his eye wear into his right eye. He tried to wash it out but had no luck.
EE was walking to his equipment when he stepped on a dirt clot and twisted his right knee.
Injured party was pushing rock in the shot, when he turned around to back up & turned back around he saw flames shoot out on both sides of the engine compartment on the tractor. Party exited the dozer & jumped off.
The Caterpillar D10L bulldozer was pushing rock in the shot. As the operator started backing up he turned back around & saw flames coming from the engine compartment on the dozer. Dozer operator got off the dozer but the fire consumed most of the engine.
Injured party was cutting a metal band around a bundle of 7"X7"X8' timbers, when he cut the band, the timber rolled over on his left ankle.
