EE stepped on platform of an isolated catwalk that collapsed beneath them causing them to fall approximately 7 feet to the next level. EE sustained superficial lacerations and bruising/soreness.

While descending a fixed ladder on the plant the EE reached the lower level and misplaced their right foot on the edge of the concrete causing them roll their right ankle.

Employee was assisting with repairs to the dredge using a cutting tool and torch to remove a bolt. Bolt head was struck with sledgehammer causing bolt to shear and fragment penetrating employee in lower abdomen. Employee was taken to ER and transferred to another hospital where employee is currently having emergency surgery

Employees were working on replacing some pipe on the dredge and while using a pry bar to pry a gasket off, the bar slipped and hit the miner in the face causing a laceration right under the left eye which required a total of 12 stitches.

Worker was driving through a ditch and EE's leg became stuck under the chair of EE's vehicle. EE also bumped EE's head on ceiling of vehicle. EMS was called due to severity of leg injury.

Employees were working on a bearing on a belt scale and noticed a fish had come up through the plant and one of the employees went over to move it. When EE picked the fish up, one of the barbs stabbed EE's hand and when EE went to throw it, the barb broke off and got stuck in EE's hand.

Two contractors were working on a chute job on one of our sites and the main contractor was in the telehandler and their EE was working on the catwalk when the operator dropped EE's water bottle and when reached to get it, lowered the boom. Causing their EE to be pinched in between the back of the forks and the stationary I-beam

An outside truck driver came onto this site to get loaded where the driver then parked their truck and was out of view from out scale house. Driver began to work on the back of the truck where the air bag blew out from under the truck and hit the driver in the head. Driver was not our employee, nor was the driver supposed to be working on their truck.

Third party contractors working for another contractor company. Welders were welding on Belly Pan of Shaker with a 6"X6" patch on outer plate of Belly Pan. Inside of belly pan was lined with rubber lining. Rubber lining ignited from contact on same plate of Belly Pan. Engulfing Shaker completely.

The EE was respooling the dragline hoist cable. EE had set the wedge in place and was holding the cable. The cable slipped out of EE's hand and caught EE's right index finger between the wedge and the cable. This created a laceration to EE's finger requiring stitches. The EE was wearing leather gloves.

An employee was fatally injured during maintenance work on the hopper. EE was operating a CAT forklift in close proximity to a Volvo excavator. EE exited the vehicle without notifying the other employees working in the area. Other employees stated they heard a "pop" and found the forklift operator unconscious next to the fork of the forklift.

A Haul truck operator was loaded coming out of the pit struck a haul truck going into the pit to be loaded. The haul truck that was hit swerved abruptly to try and avoid being hit. The operator that was hit suffered low back pain, prescribed PT, pain meds and muscle relaxers. Damage to front left of one haul truck and both tires on the other.

Employee operating loader under supervision when EE became stuck in sand stockpile being jarred and bounced causing lower back and tailbone pain, no report of any injury until 12/7/20, sent to clinic, prescribed lodine, terminated 12/7 for non-reporting

The EE was moving the swing cable to gain slack and make a connection to the anchor point, they tripped on uneven ground. The cable was in hands, waist level when they tripped. EE dropped it and caught themselves with their hands.The EE felt immediate pain in right shoulder. EE was taken to the doctor and prescribed pain medicine, muscle relaxers, and physical therapy.

The over the road gravel hauler was helping a coworker dump overloaded material near the scale house by letting the coworker know how much to dump. After dumping, employee reached hand in to latch the tailgate when hand got caught & lacerated requiring stitches. Employee did not realize it had an auto latch tailgate unlike employee's truck which has a manual latch.

EEs were performing a full log washer paddle change, all were properly LOTO in the MCC. Logs were secured with a come along to prevent the weight of new paddles to spin the logs. The come along slipped, EE became pinned between paddles and the wall of the log washer, EE was freed quickly. The accident caused abdominal contusions, EE was prescribed pain medication and light duty.

The employee was cutting a stockpile with a front end loader when a large chunk of frozen road gravel fell from the top of the stockpile onto the front of the loader and into the cab. The employee suffered minor cuts on the right hand and lower back pain from removing material from their lap in order to exit loader.

The EE was attempting to remove cutting blades from the back of EE's truck, with mobile crane. EE had a pry bar in between the blades EE was holding the top blade up with right hand and trying to put strap around with left hand, EE had leather gloves on. The blade slid back and fell on left index and middle finger causing bruising, and swelling. No medical attention needed.

Emp claimed inj back on Fri. 7/28 but did not report it until Mon. 7/31. Emp was oper. a Komatsu 400HM haul truck and hit a pothole in the haul rd. causing inj to lower back. This report was originally submitted via paper copy, and this report is being generated to make amendments per MSHA field office. Original was submitted 8/8/17.

Employee was attempting to remove a large mud ball off the main feed belt. While removing the mud ball he suffered a lower back strain. He went directly to his immediate supervisor and notified him of the incident and was taken to Columbus ER where he was treated and released back to light duty which will cease 10/11/2015.

EE fainted while at work. He was transported to Columbus Community Hospital via ambulance & treated at the emergency room. The cause of fainting was determined unknown by the ER doctor. EE followed up with his regular doctor the next day & was prescribed anxiety medication. He has not been allowed back to work due to unknown side effects of prescribed medication.

Employee suffered an injury to her neck/rotator cuff. She attempted to reach across her body and behind her to retrieve a paper and her chair came out from underneath her and she grabbed the wall to break her fall and suffered a shoulder injury. She reported the incident to her manager but refused medical treatment. She went to the doctor on 8/10/15 for treatment.

Employee (miner with 9 years experience) was loading an empty on road diesel tank onto a truck to be removed from site. While loading the tank on the tommy gate of the truck, his hand came in contact with sharp area on the containment tank and lacerated his hand. The laceration required 5 stitches to close and employee also received a tetanus shot while at the ER.
