MINER WAS SPOOLING IN A WINCH CABLE. EE WAS GUIDING THE CABLE WITH LEFT HAND AND CONTROLING THE FEED WITH THE CONTROLLER ON RIGHT HAND. AS THEY WERE ALMOST AT THE END, A U BOLT THAT WAS USED TO CREATE A LOOP AT THE END OF THE CABLE, CAUGHT THE CUFF OF THEIR GLOVE AND PULLED THEIR HAND INTO THE OPENING, THE U BOLT CAUGH THEIR MIDDLE FINGER AND LACERATED IT.

Showing all 4Miner was working on top of a water tank that was approx. 7' tall. Miner was removing an Air Compressor that inserts pressure into the water tank. As miner proceeded to remove it, the compressor suddenly propelled forward with enough force to strike miner on the face and chest area. Miner fell sideways to the ground hurting right leg.
Employee believes he was bit by a spider on 06/18/15 while turning on water pump. Employee did not inform supervisor of bite until 06/19/15 employee did not want to receive medical treatment. On 6/23/15 the location had become infected employee was sent to receive medical treatment. Employee has been and remains on Full Duties.
While performing maintenance on pump 904 employee was assisting with re-attaching the discharge hose to the pump when his right thumb was injured when pinched between the pipe and hoist. Injury consisted of fracture and laceration (2 stitches) to tip of right thumb.
