Employee was removing a steel plate (16"x 16") with the assistance of another employee, one was holding while one was cutting, when the steel plate was freed up the employee was in an awkward position and tried to handle the plate by himself. Resulting in a strain to his right arm and was given work restrictions returning to work.

Employee was walking to the stacker belt after a heavy rain storm, there was an accumulation of water he tripped fell forward and hit his head on the metal frame, leaving a red mark, there was no excessive swelling, no loss of conscience.

Employee was working in a dozer when he decided to exit the vehicle. At this time he caught his left foot on the foot rest and twisted his left knee. Employee hurt his knee on the 5th and sought medical attention on the 6th of November.

Employee did not report until 8/19/13 after his supervisor asked why he was wearing a brace on his wrist. He stated he injured it on 8/14/13 not really sure how it was just sore. At this time he did not seek medical treatment. On 8/21/13 the employee went to the Urgent care they diagnosed him with tendonitis and told him to wear a brace at all times and gave him restrictions.

Employee was bending over to change out screens. He felt sore afterward but did not report until 12/10/2012. His doctor placed him on restricted duty.

Employee was aligning a gear box on a bucket elevator, and as he pushed forward he felt a pain in his lower back.

Employee was picking up an extension cord in the maintenance shop. When he turned around he bumped his knee into a table, and his knee started to swell up.

Employee was cleaning out spray nozzles using a small pocket knife when he cut his hand.

The employee slipped on ice in the parking lot that was built up under a layer of snow. The employee injured his ankle and is on restricted duty.

Employee was lifting a 5 Gallon bucket of oil weighing approximately 35 lbs. When she lifted and turned the employee felt discomfort in her lower back.

When adding extra water from the City and Plant, in an effort to fill pond three for the first time. It was believed that the additional water being pumped into pond one overflowed and washed out the South East corner of the Pond. No water entered the Plant or Mining areas, the water traveled East across Exel Easement into a wooded area and a farmers field.
