Employee was tasked with loading a 22lb pump into a man basket for transportation to an underground heading. Employee lifted pump alone, and sustained an injury to her back. Employee was diagnosed with acute lumbar back sprain, she received x-rays and an office visit. Employee to have follow up on 4-09-13 and may be tasked with light duty on 4-10-13.

Employee was operating haul truck in intersection, employee checked behind to ensure truck would clear electrical installation. As employee proceeded forward through air curtains, dirt/debris was flipped up into employee's right eye. As a result of this employee suffered scratches to right eye.

Employee claims to have hurt his back while operating a haul truck. Road was rough and seat suspension launched employee upwards hitting his head on the ROPS causing pain in upper back. Began hurting a week later when he went to the ER room on 01/09/2013. That is when accident report was received.

Miner was climbing up a raise to clean down muck from previous blast. Miner was up about 30 feet on the ladder when ladder swung away from the rib. Ladder had loosened up from precious blast and allowed muck to fall from above about 8 feet and struck miner on left shoulder, arm, and back. 12/17/12 placed on authorized time loss until MRI in 2 weeks.

Jack-leg drill had a worn out chuck and employee was showing the mechanic the problem with the chuck. When employee set the drill back against the face he had his hand on the drill throttle control and a small rock fell from the face and landed on the side of his thumb fracturing his thumb.

Employee attempted to bend over a piece of hog wire with his hand to prevent it from snagging on someone. Wire sprung back and punctured palm of left hand through employee's glove. This resulted in employee having to go to the hospital ER and having wound cleaned and 2 sutures to close wound.

Employee was lifting a box of resin cartridges onto a shelf about 40 inches from the sill felt a sharp pain in back. Was taken to hospital and placed on modified duty until the results of an MRI are reviewed.

Employee was drilling a face round with a jackleg drill. The leg on the jackleg kicked out from the sill and the D handle on the jackleg struck the employee on the lower left leg. Two weeks later on 06/05/2012 employee went to a doctor and had the fluid drained from the bruise.

Employee was operating an MTI 210 LHD, while in the process of turning left, employee had his right hand on top of the gauge console which is a pinch point. This resulted in the employee smashing his hand between the FOPS and the corner of the gauge console. This resulted in employee receiving 2 stitches to the puncture wound.

Employee was cleaning out a diesel fuel line with brake cleaner. While spraying the fuel line with the brake cleaner the cleaner ricocheted back into the employees face. Brake cleaner also went up under the employees safety glasses and into both of employees eyes. This caused a burning sensation, pain and loss of vision to both eyes

Employee was operating an underground L.H.D. loading haul trucks. Employee hit a bump in the travel-way and the D-Ring on his miners belt got between him and the back of the operators seat. This caused the D-Ring to strike the employee in the lower portion of his back causing pain and numbness.

Employee was holding a 6" tailings decant water line that had burst at a splice, and was trying to direct the water towards a small water impoundment. The water line was to heavy for the employee and he dropped the hose. When he did so the banding from the splice cut his right hand ring finger.

Employees left foot was caught in hoses, employee jerked up with his left leg to free it from the hoses and struck his knee against a ladder. Injuring his left knee. It was latter found out that employee had surgery on his knee, but had not informed anyone at the mine site. Employee has been released to modified duty but has chosen not to return to work.

Employee was pulling Femco Telephone cable from a cable reel. Cable hung up on reel, employee pulled down hard on the cable and cable suddenly came free. Employee lost his balance and fell down to the ground striking his head below his hard hat. This resulted on a one and one half inch laceration to the back of the employee's head. EE received six staples to close the wound.

Miner was drilling large diameter up holes with a jack leg drill. The jack leg drill steel hung up in the drill hole causing the jack leg drill to spin. This put the miner into the rib wall injuring his left shoulder. Miner reported accident but continued to work for 5 more 10 hour shifts. On miner's 7 days off he called in & reported that he had gone to dr about shoulder.

Employee and another employee was drilling a face round. Employee was closer to the face than the other employee, when the drill steel broke about 6 inches from the chuck of the jackleg. Employee operating the jackleg could not control the jackleg and it punctured the employee closer to the face in the left foot. Resulting in 2 stitches to close wound.

EE was using a come-along to hoist up a large battery charging station. He was pushing the suspended load and was releasing the come-along down. When it released, his right arm got punctured by the leg of the charging station.

Miner was operating a jackleg drill bolting up the heading, when miner was ready to pull 8' steel, nipper (helper) pulled back on the 8' steel at the same time. Miner put his hand under the drill and smashed his hand between the drill stop and jackleg.
