Maintenance worker had been cutting, welding, and preforming regular maintenance duties throughout the day. Employee was perspiring and used sleeve to wipe perspiration from cheeks. Few minutes later EE felt like eye had something in it. EE washed eye out, but eye was still irritated. Took employee to Doctor. Eye was scratched. Rx given to prevent infection.

Employee was changing cutting edge on skid steer. Had hand placed on nut when ee used impact to loosen bolt, causing nut to cut through glove. Taken to ER received stitches.

Employee was using a wrench to hold the head of a bolt while tightening with impact. Felt a pop in employee's left bicep area. Taken to Doctor same day, returned to work without restrictions. Returned to primary doctor, returned to work with restrictions on 7/28/17.

Employee was pulling wire into MCC room. While pulling wire EE struck the back of hand on a light fixture. This caused a gash above, middle finger knuckle. Taken to doctor 6\20\2016. Doctor stated tendon had been severed. Returned to work with restrictions same day. Doctor referred employee to specialist. Employee retired as planned on 6\23\2016

Employee was entering MCC room. When switching his weight to his right leg his foot slipped out from under him. He placed his right arm out to catch his fall causing the injury. Returned to work without restrictions 3/24/15. Returned to doctor for evaluation. Reportable as of 4/22/15 for medical treatment. Physical Therapy ordered for tendinosis.

Haul truck driver was performing normal task when he felt an irritation in his eye. After the use of eye wash station employee stated his eye still felt irritated. Taken to doctor as precautionary measure. Prescription medication prescribed, returned to full duty.

10/17/13 - Employee finished installing rollers on conveyor. While walking back from task, stepped on rock and strained Rt. ankle. First-aid was administered but employee had difficulty walking. Transported EE to Dr. X-ray negative. EE given soft ankle support and restrictions. 4 days restricted. Reportable as of 10/17.

MRI ordered for 10/15. Dr. reviewed MRI with shipper 10/16 - released with restrictions. Physical therapy ordered for tendonitis Rt shoulder. Reportable as of 10/16/13.

On 7/25, employee slipped while using a pry bar, striking head on structure. Employee was given first aid and offered medical exam, employee declined. On 7/26, employee requested medical exam and was seen at ER. Diagnosed as mild concussion given days off work starting 7/27. Reportable as of 7/27.

On 2/28/13, employee was lifting return roller to install on BC-1. Noticed pain in back, a few hours later. Reported incident to operator 3/4/13 as back pain but did not want medical treatment. Employee went to ER 3/11/13 and received prescription medication and restrictions. Reportable as of 3/11/13

Pulling hand rail between domes and the wind pulled on the hand rails. Hand rail caught on conveyer trying to pull it off. Note he didn't go to the doctor until 8/23/12 during Hurricane Isaac.

REDLINE - Contractor Employee (Fritz Construction) was cleaning the track on an excavator when he felt a sting on his chin, raised pant leg and found an ant. Contractor noticed redness and inflammation on Monday 7/16. Contractor went to Doctor on Tuesday 7/17 and was given prescription medication for infection. Operator notified 7/17/12

Redline. Contractor employee was assigned to assist contractor personnel applying shotcrete (gunite). Task was to move hoses filled with gunite. Injured contractor complained of lower back pain and reported injury 6/14/12 Was seen by physician 6/14/12. Given prescription medication and restrictions. Contractor employee has not returned to work.

REDLINE - Contractor employee (South Industries) slipped, but caught himself while carrying rebar. Later stated his knee was bothering him. Employee went to doctor 5/10, X-ray's and prescription medication were given. Missed two days work. Accident was brought to our attention on 5/11.

REDLINE - Contractor employee (Fritz Construction) was working north of storm water pond when he noticed leg was itching and irritated on Wed/4/11. Employee was sent to doctor by Fritz, Fri/4/13. Infected spider bite was diagnosed. Prescription medication given for swelling / infection. Returned to work Thur/4/19. 3 missed days. Operator notified 4/16/12

Employee was climbing down steps of loader, while stepping off last step his foot was on uneven ground, which caused him to twist ankle. Employee was taken to doctor. Sprained ankle and prescription medication given. Employee's work activities were restricted to prevent aggravation of injury.

Employee was cleaning muck at Wet Plant with a shovel, near BC-10. Employee states windblown sand entered his right eye, although he was wearing safety glasses. Employee did not report the incident until 3/8/12 at 6:00 AM. employee taken to doctor. Scratched cornea, prescription medication, returned to full duty.

EE reportedly wiped sand in his left eye about 7:00 PM and did not report this incident until shortly after 3:00 PM on 01/07/12. EE reported the incident to his Supervisor. Supervisor directed EE to the local Emergency Room for treatment. The E.R. discharged him with prescription medication, an eye patch and instructions not to report to work until Monday, 1/8/12.

On Oct/26/11, an employee reported he hurt his elbow on 7/6/11. He was pulling on a wrench, it slipped and he struck his elbow on a Rotex screen. Employee was seen by a physician Fri/Oct/28. Physician prescribed steroids. X-Ray negative for fracture. Physician informed operator by phone Mon/10/31/11. No permanent impairment expected.

Employee was washing out beneath sand dryer with a 1.5 inch water hose. Employee attempted to shut off hose using an adjustable wrench when it slipped from his hand, with the nozzle striking the employee underneath his hard hat, just above his right ear. The laceration required 1 staple to close the wound. The employee was released for full duty that shift.
