Showing all 7Employee was walking across the yard and slipped on a wet area and twisted ankle. Employee reported pain in the ankle. The yard was level and clear except for the wet area where the employee slipped.
Employee turned slightly and bent over to pick up cable and felt pull/pain in lower back
Employee was helping to unload pipe, stumbled, fell on ground, hurting hip. Employee reported incident to supervisor but was OK and continued to work. One week later employee complained of pain in hip. EE was sent to doctor for examination. Dr found no problems prescribed a cortisone pill for one week and sent employee back to work no restrictions or lost time.
Miner exited excavator to break for lunch on 6/29/15 and stepped down into a rut. Miner felt a pop in his back. Miner completed shift. Miner reported to work at 4:00AM on 6/30 and performed normal duties. At approximately 7:00AM on 6/30, miner reported injury to supervisor.
Was grinding pontoon when grinder caught on steel and jumped up and cut his chin
Employee was cutting a gasket using a sheetrock knife not wearing gloves and cut all the way through the gasket cutting the palm of his hand.
