Employee was shoveling and cleaning material when EE felt a sudden pain across EE's back. The employee had previously injured EE's back in a wake-boarding accident.

EE was helping another employee change screens in one of the rotex screeners. As the two employees lifted and slid the bottom deck screen in place, EE felt a sharp pain across EE's entire back.

The employee was using a hand crank to raise the crossover platform on the rail load out. EE then engaged the stop gear on the crank. As EE released the handle the stop gear did not engage, this caused the handle to free spin. The handle struck the employee on left index finger, the impact of the handle cut finger. EE was wearing gloves at the time of the incident.

Employee was walking across railroad tracks in the rail yard. As the employee crossed the final track on the way back to the truck, employee fell and hit head.

EE was moving a rubber mat in the mine area. EE tripped over the mat its self or the uneven terrain and fell awkwardly on to right leg. EE had pain in right knee and slight pain in right ankle.

EE was operating an articulated haul truck. While being loaded by a 980H CAT wheel loader the truck EE was operating rocked and was shaken by the material being loaded. This caused EE's neck and shoulder to move in an awkward motion causing pain and discomfort. EE then was experiencing dizziness, headaches, stiffness, and pain in the legs.

Employee was working on installing a screen in one of our screener. He Claims he was struck in head with the rubber mallet that bounced off the screen after the other employee hit the screen. They were installing the bottom level of the 2 level screening deck. All PPE and lockout procedures where followed.

The employee was assisting in pulling a new electrical line for our well. He was bent over and when he stood up his back was tight. No one else was injured. He was wearing all of his PPE.
