Injured employee was tack welding small metal parts onto the wall of a loader bucket. After the tack weld, the employee placed a hand on a wear bar to assist themselves to stand up. As they stood, the tack weld failed and allowed the part to slide down approx. 12 inches and contact right index finger against the wear bar. This dislocated right index finger at the first knuckle

A mechanic was removing GET from a loading unit and while removing the last bolt with an impact gun, the mechanic reached underneath the bucket and they cut their hand on the head of the bolt.
