At around 8PM on January 22nd, the Jet Mill Bagger was attempting to cut open a pallet of new bags and slipped causing a finger laceration that required stiches to repair. The bagger was using a box cutter to cut off the plastic banding used to secure the pallet loads.

Employee was removing a bearing from a pump shaft. The employee cut the bearing in two places then used a hammer and chisel to separate the races of the bearing. When hammering on the bearing race, a small sliver from the bearing housing broke off and struck the employee on the left wrist.

Operator was walking down a flight of stairs and sprained right knee. EE was not carrying anything, rushing and was using the handrail. EE was treated with prescription medication and physical therapy and given work restrictions.

The operator was hammering the locking ring handle on the discharge valve of the south EW pump. When the operator struck the locking ring and it broke free, the turning handle spun striking the operators right index finger holding the hammer. This impact caused a "crush injury" right above the nail bed.

Operator was shoveling material off of the mezzanine at the bulk loading area into a drop chute when they sprained their back. Reported incident to the supervisor Thursday 3/10/22 but didn't request medical attention until Friday 3/11/22. Diagnosed with back sprain and given medication and restrictions.

Employee didn't follow procedure and attempted to clean the bulk loading belt while in operation. Employee's hand was pulled through the pinch point between the troughing idler and the conveyor. Right hand bone broke.

Employee was shoveling dry material from the ground. About 10 mins into the process, complained about shoulder hurting. Medical evaluation resulted in restricted duty for 3 days. Huber does not have a light duty system, results in LTI until 100% released.

A B45 Operator was dumping the presses into the dump truck. The Operator backed the truck under the second press, turned off the truck and in the process of stepping out of the truck twisted an ankle. The truck was parked rather close to the containment curb and the Operator stepped on the curb while exiting.

Employee reported that EE has been experiencing pain in the groin area for about a week. Nothing was reported to management until 3-26-2019. EE was sent to the company doctor for evaluation. The treatment: no lifting more than 20lb, and a Rx for pain. Our site can not accommodate the restrictions, Thus the injury will result in an LTI until a full release from doctor.

Employee reported that they had slipped on flat, dry concrete causing an ankle to turn. The employee was treated at a local clinic for a pulled muscle and returned to work with restrictions. Company policy prevents employees working with restrictions, so employee was sent home until they receive a full release from the doctor.

Operator was placing an empty super sack on a forklift. During the act of pulling the loops in place employee felt a sharp pain in the right shoulder. Dr. determined the injury to be rotator cuff strain.

Employee was cleaning a filter and got caustic slurry on pants leg. The employee decided to put on rubber boots and tucked the wet pants leg into the rubber boots. The caustic caused the employee to receive 2nd degree burns.

The employee was working on tank 69/70 conveyor head pulley. To get the motor and gear assembly out from under the platform they had to use a pry bar so they could chain hoist it out. While moving the assembly EE felt a pop in right forearm. The pop was instantaneous and EE did not stop working. Employee did not want to see Dr until 3/22 they referred to specialist on 4/7.

Employee was removing the east end of slurry heater. Employee dropped a nut and when the nut hit the floor it landed in some product and it splashed into the employees right eye. Employee was wearing safety glasses, but he had removed his goggles after the first break had occurred. Employee used eyewash and saw Dr who prescribed antibiotic eye drops as a precaution.

On June 9, 2012 at approximately 6:40am an employee stepped in a puddle of caustic slurry. The employee did not notify supervision of the incident until Tues June 12th. The employee received burns to both of her feet. Any notification of supervision within approximately one hour of contact with this weak caustic would have prevented a recordable and lost work day.
