The employee was pulling on a wrench and the wrench slipped hitting the employee in the bottom lip and front bottom tooth, causing a small laceration to the lip and a small chip to the tooth.

EE was on the 4th step of an 8 foot ladder, drilling on a lube reel box to install a buggy whip. The drill bound up causing the ee to lose balance and fall to the ground. EE was seen at the ER, diagnosed with a fracture to the radial bone. EE was released to restricted duty.

The Operator looked out the side door of the Forklift EE was operating and as doing such, EE struck upper front tooth on the hand held bar that assists the operators while entering the cab, and broke part of the tooth off.

EE dismounted water truck and walked towards water stand. Feet got stuck in mud and ee fell forward to the ground, catching self with both hands. Seen at ER where right wrist fracture was diagnosed and the ee was put off work pending consultation with an orthopedist.

Ee was removing driveline from a D10 dozer. While removing the bolts from the U joint fasteners, the driveline pivoted pinching the ee's right thumb between the yoke and the driveline. Seen at ER, diagnosed with a fracture and a laceration requiring stitches. Released to restricted duty.

Ee's haul truck was being loaded. A large boulder was placed in the bed of the truck, causing the ee to be jostled in the cab. EE reported to EE's supervisor, was seen by the EMTs and refused treatment. On February 18 EE sought treatment with a chiropractor and filed a WC claim. Investigation completed and claim was accepted 03/28/19. Reporting for medical treatment.

EE was walking ribbons during metal removal. EE states was walking a ribbon and stepped down funny because the ribbons are not level, EE felt pain in the knee. EE was seen at the ER and diagnosed with a strain to the knee and put off work.

EE was opening an air door on a motivator and it "sprung" open unexpectedly, striking the EE on the left shoulder, knocking them to the ground. Initial xrays did not show dislocation, but subsequent follow-up found the shoulder was dislocated. The EE is released to restricted duty.

While working on a cooling pump of a 4 yard loader, the access door came unlatched hitting the employee on the back of EE's hard hat. When hit on the hard hat the force thrust EE forward causing EE to hit EE's nose on the loader frame.

The employee stepped off of a concrete step and twisted their right knee.

Ee was carrying a box on shoulder when it started to slip. Ee reached with right arm to catch it and pulled shoulder. Ee was seen at the clinic, diagnosed with a right rotator cuff strain, and released to restricted duty which the site did accommodate.

Ee was doing a final walk around of equipment at the end of shift and twisted ankle. EE was seen at the clinic the evening of the 10th and put off work pending follow-up. EE was continued off work after follow-up and diagnosis of a severe ankle strain.

Truck driver was jarred while being loaded and reported a back strain.

The haul truck operator was getting loaded by a shovel in the CHOP pit. The operator experienced pain in the back and neck area after the third bucket was placed in the truck. Ee was seen at the ER and subsequent follow-up at Golden Health. The ee was put off work and started in physical therapy.

EE was getting into crew van and hit knee on the door. EE felt immediate pain but continued to work. EE worked 4 more shifts then sought treatment on 09/11. Doctor diagnosed EE with a fractured left patella, gave EE a soft brace, and released EE to full duty.

Employee was positioning a HILTI Hammer-Drill inside the re-bar to be able to drill the next hole for re-bar when EE's left hand made contact with the trigger engaging the drill while right hand was on the bit. EE's glove became entangled in the bit causing dislocated fingers and lacerations on the right hand.

Empl. was being loaded when a rock dropped shaking the truck. Next day at 9:00 am empl. complained of neck pain. Empl. was evaluated by Physician & released to work with cautions to avoid "jarring and neck motion". Empl. chose not to return to work at this mine and has been permanently transferred to another non-mining job.

Ee was removing drain plug with a ratchet and the ratchet slipped, pinching the ee's left pinky finger. Ee seen at ER and sutures were required. Ee released to full duty.

Ee was working on radiator grill, it slipped and struck the machine, and hit the ee in the right ear. Laceration required stitches. Ee released to full duty.

Ee was welding and twisting when ee felt numbness in leg. Ee was diagnosed with sciatica and muscle spasm and released to full duty. Claim reopening requested and approved 03/19/19. Conservative measures failed and ee required medial branch blocks on 11/04/19 making this reportable for treatment.

2 Labors and skid steer operator were installing French drain around portal sets. Labor was holding fabric told the operator to back up while that was happening labor started to stand up while operator was tilting bucket back and pick hand between bucket and wall.

Ee was backing up forklift with door opened and latched. Ee had hand holding door frame; strong wind blew door shut, pinching the ee's right thumb between door and frame, causing fracture to tip and laceration. Ee required stitches and was released to full duty.

EE was driving underground in open cab. Ventilation blew some fine dirt into an eye. Employee rinsed the eye and continued working. The next day the eye was irritated. Employee was seen at the clinic and diagnosed with a corneal abrasion. Employee was given prescription eye drops and released to full duty.

While adding 12' screen casing, the casing being put into the derrick hung up on the elevator ring of the previous piece of casing at the table and the casing being added bounced up striking the employee on the point of the jaw.

EE filed a workers' comp claim for possible carpal tunnel on 04/23/18. Claim denied pending a medical investigation. Medical reporting received on 08/28/18 by comp carrier, carrier confirmed that the claim will be accepted on 09/15/18, making this reportable for a diagnosed and confirmed cumulative trauma. EE will require surgery, but is working full duty at this time.

Ee was moving metal fittings with two coworkers. As EE bent down and began to lift, EE felt a sharp pain in EE's low back. EE stopped the lift and was taken to the ER for evaluation. EE was diagnosed with an acute muscle strain and spasm, and was released with restrictions that the site could not accommodate.

Ee was dismounting grader to continue pre-shift inspection. EE lost grip with right hand, lost balance, and fell to ground from 3rd step on ladder. Seen at ER where ee was transported for surgery on an open fracture to right ankle.

Ee was dismounting haul truck and "hopped" down from the last step, landing on right foot. When ee landed, ee felt pain in foot. Ee worked next night shift and went to the doctor the following morning. Ee was diagnosed with a fractured bone in right foot. Released with restrictions that site could not accommodate.

Ee was cutting core, placed rock under running blade. Saw pulled rock and hand into blade, nicking the left index finger knuckle. Stitches required and ee released to restricted duty.

Ee was working with another ee, had swellex in vice and coworker was using wrench to spin it. Injured ee's hand was caught between the vice and the swellex. Seen at ER, diagnosed with fractured fingertip and laceration. Stitches required and ee put off work for 2 days, then restricted.

Miner applying shotcrete to newly excavated portion of the east decline tunnel for initial ground support. Shotcrete nozzle plugged causing the supply hose to jump which caused the miner to lose footing and miner fell striking the ground with the right elbow dislocating it.

Ee was climbing a ladder to remove a trip cable at 351 shovel located at pipeline. Bango arm swung around and struck the ladder, causing the ee to fall approximately 5 feet to the ground. Ee landed on left side. Fractured left arm, bruised left knee, strained left ankle. Off work pending an appointment with orthopedic surgeon.

Employee energized air hose, the opened end of hose whipped around and the hose fitting struck employee on the head. The hose caused a laceration to the employee's head. Employee was released to full duty.

EE was pushing wheelbarrow through mud and it tipped over. EE went to brace themselves and landed on their right thumb. Fractured right thumb. EE released to restricted duty which the site could accommodate.

Ee was pulling bolts, one of the bolt heads came loose and spun, cutting EE's thumb. Seen at clinic and stitches were required to close the wound. Ee released to full duty.

Ee stepped off dozer, foot caught in grating and ee hyperextended left calf/Achilles when ee landed on left toe. Seen at ER, diagnosed with strained left calf. Follow-up confirmed diagnosis, ee released with restrictions site could not accommodate.

Contract employee was taking a mop bucket out to the janitor closet outside of the dry. EE slipped backward on the wet floor and fell, and was struck in the face with the bucket. EE was transported by light vehicle to NNRH with a cut over the eye. Subsequent follow-up at University of Utah resulted in stitches and 3 days off.

Ee was rolling up a cable reel and left thumb became caught and was crushed. Seen at ER, diagnosed with open displaced fracture of distal tuft, distal phalanx of thumb. Referral to orthopedic surgeon resulted in surgery. Ee lost time.

Ee was pulling on hose and it broke free, striking ee in the mouth. Seen at the dentist, diagnosed with 3 broken/chipped teeth, one of which had to be extracted. Ee released to restricted duty for two days.

EE has been a maintenance mechanic for many years and works on knees frequently. Has had increasing knee pain. Diagnosed with bilateral patellofemoral arthritis exacerbated by work activities. Accepted as work related on 04/13/17. EE off work due to occupational disease.

Interwest Supply employee (contractor) was working with a Barrick employee unloading 2x2 steel tubing from forklift onto steel rack hitting contractor on the forearm causing a cut. The contractor was seen at the ER and stitches were required to close the wound. EE was released to full duty effective 03/20/2017.

Ee was cleaning an anode when it started to slip. Ee reached for the anode and cut EE's hand on the sop edge of the anode plate. Seen at the clinic and stitches were required to close the wound. Ee released to restricted duty which the site can accommodate.

Two Employees were changing the isolating mounts on the VFD housing on a drill rig and using a come-a-long to lift one of the corners. The come-along broke and the VFD housing dropped down and contacted the socket and ratchet. The ratchet shifted and pinched the employee's index and middle finger in between the ratchet handle and rig substructure.

Employee was changing the position of the return hose flange and when the flange came off the last bolts holding it, it slid back and pinched thumb between flange and cyclone brace.

Ee was driving up ramp in haul truck, and another haul truck driving down, slid and made contact with passenger side of ee's truck, jarring the ee in the cab. Seen at the ER, given meds and released to full duty. On follow-up on 01/11/17, ee placed off work.

Employee walked into water to shut off a valve and felt lt. knee "pop"

Crew was loading compressor on semi-truck trailer and employee's left hand was pinched in-between trailer bulkhead and skid mounted compressor.

Ee has operated the shovel for 20 years. Is now experiencing severe pain in both hands and wrists. Seen by doctor on 12/16/16 and diagnosed with carpal tunnel, both hands, due to work duties. Released to full duty.

Ee was replacing left rear suspension, left finger caught between collar and axle box frame. Sutures required and ee released to restricted duty.

Putting lifter on BQ pipe and sub with lifter tipped over catching employee's little finger between the sub and the BQ pipe.
