A welded connection broke on a metal support for a feed box. When the weld broke, the feed box shifted and the hose feeding the feed box came out of the inlet pipe and fell toward the ground. A miner was standing under the pipe at the time and as the pipe fell, it struck the miner. The pipe struck the miner's hard hat and back of the head at the base of the neck.

Employee was changing a gearmotor for the mill trommel. The gearmotor was located on the floor. The employee, along with one other employee, lifted the gearmotor from its mount and moved it out of the way to make room for the new gearmotor to be installed. The next morning, the employee reported lower back pain and was refered to seek medical care.

Employee was attempting to start pickup to warm up. Engaged clutch with right leg while standing with left leg on the ground. Released clutch. Truck was in gear, lurched forward, collided with dumpster and slammed driver's door into employee's body. Employee received a laceration to upper lip, requiring stitches at local ER.

Employee was on the way to check reagents and tripped over a frame or piping that was left in the walkway.

The worker was unspooling the cable and walking it back toward the load-out area to connect to a railcar. EE felt a pain in EE's lower back.

The employee unplug a line on the #8 spiral when EE step on an drain covering. The covering flipped tripping the employee foot. EE stumbled twisting back awkwardly catching balance to keep from falling.

New extractor belt hoses not secured in final position. Employee inspecting motor installation, stepped down to work platform, tripped over loose hoses, fell to work platform.

Contractor was unloading dirt from Hannah Mine #1175 at Specialty Vermiculite where the box tipped over on the right side. It is believed that contractor driver attempted to dump on uneven soft ground. Accident resulted in minor chest injuries to contractor driver. Chest wall contusions, 7th rib, physical damage to trailer box.

Metal floor grate panel was removed for cleanup of material underneath. Employee stepped into open hole created by panel removal, losing balance, twisting and straining leg as they caught themselves while falling.

Employee was lifting fork of extended reach lift to adjust width of forks. EE dropped fork into place. EE's hand position allowed the tip of pinky finger of EE's right hand to be crushed between back of fork and support cross member of backstop.

TRUCK OPERATED BY CONTRACTOR WAS DELIVERING LOAD TO MINE. DRIVER DID NOT SEE TRAIN. TRAIN STRUCK POWER UNIT ON DRIVER SIDE. DE-RAILED ENGINE AND 2 TANK CARS, DRIVER'S WIFE TOOK DRIVER TO HOSPITAL AFTER SCENE CLEARED TO BE CHECKED, WAS TREATED AND RELEASED TO FULL DUTY FOR THE FOLLOWING DAY.

A miner sustained an injury to the back of their right hand when their glove became caught in a conveyor and pulled their hand in. Damage and removal of tissue on the back of the miner's right hand was caused by this incident.

Employee temporarily removed grating covering a pit to clean out the pit. Employee backed up while unrolling a vacuum hose and accidentally stepped into the pit. The employee fell against I-beam over the pit, striking right shoulder. Impact from the fall caused dislocation of the right shoulder.

Customer truck driver sustained minor burns to the hands and face and swallowed a small amount of antifreeze when checking the coolant levels on their truck, which was running hot.

Operator was dumping load on soft, uneven ground. Load shifted while bed was raised. Truck slowly turned over onto side. Operator exited cab under own power.

Employee was working in an electrical panel with live 480 volt feed coming into unit. Employee was working alone, with minimal PPE. Employee did not try out or test circuit for current before beginning work. Employee was exposed to arc flash in close proximity to panel, experience burns on face, ear, left arm and hand.

Operator was doing the routine end of week equipment shutdown and clean out when he got a chemical into his eye. Investigation determine there was a small drip from the line one floor higher. Employee was wearing all required PPE including safety glasses.
