A strap was being used to check the pivot bearing on a grader. The strap broke and struck the employee in the left hand. Ligaments in the left hand were weakened and the employee was released on light duty.

The employee was exiting a pickup and tripped and fell out onto the ground. The foot was tangled in wiring fed through the passenger door. After medical review it was determined that the right ankle was fractured and that EE would need to wear a boot for three (3) weeks.

Employee was operating a Cat 793D haul truck and was being loaded by a L1400 Letourneau loader. The loader put the third bucket in the truck when a rock rolled to the right side of the truck box and caused the truck to rock which caused the employee pain in the neck area. Employee was seen at the local clinic and released to work. The employee missed their next scheduled shift.

EE notified supervisor at end of shift their lt. eye was irritated. EE claimed that during safety meeting at start of shift something got into eye & over the shift caused the irritation. Supv. offered to have EE seen by mine rescue & get eye flushed. EE refused treatment at mine; however, sought med. attn. after shift. There was nothing in eye to remove but cornea was scratched.

Employee was working on the explosive crew and was carrying a 35 pound box of boosters across the pattern and felt a sharp pain in their right knee. The employee has managed the injury but required surgery on 1-29-2019 which is also the first missed shift for this employee.

Employee was using a hand press to remove a u-joint from a light vehicle driveline when the driveline shifted and contacted the employee under the chin. The laceration required 3 stitches to close.

As employee was climbing the boarding ladder on to 59 haul truck they contacted their knee with the boarding ladder attachment. Employee later called the supervisor reporting that their knee was injured and their neck was sore.

Employee was repairing exhaust on a D10T dozer and strained their right wrist. Physician released employee to work. Physician prescribed a support brace on 11-22-2017 and the wrist required surgery on 1-16-2018.

Employee was removing a carrier plate off of a D11 dozer and had their left thumb pinched between the plate and the swing frame. The employee pulled their thumb free and noticed it was bleeding. Local medical center doctor advised employee to come in after the shift. The wound took 2 stitches to close.

Employee was removing a wheel chock from the bumper of a haul truck and felt a pop in their right shoulder. Employee was seen at local medical center and diagnosed with non-surgical tears in the rotator cuff muscles.

Employee reported to line-out meeting at beginning of the shift and was attending a safety presentation that was to be 1 1/2 hours in duration. 30 minutes into the training, the employee was not feeling well and was going to go get some fresh air. The employee stood up and fainted and dropped to the floor. This resulted in a non-displaced hairline fracture of upper jaw.

An employee was kneeling down testing the voltage on a switch, when employee stood up, employee felt a pain go down back. MRT was dispatched and employee was taken to the hospital for evaluation.

While working on a dozer, a sledge hammer slid off the final drive of the dozer and smashed the employee's finger between the hammer and the dozer swing frame. The injury required 4 stitches.

While moving chock blocks and pulling on a hose reel, an employee had a sharp abdominal pain. Employee was taken to the hospital for observation. It was determined the employee had a hernia that will require surgery.

The fuel truck was going up a ramp. The lights of a passing haul truck made it hard for the operator to see. The fuel truck hit the berm on the edge of the haul road and tipped the rear of the fuel truck onto its left side. The operator cab portion stayed upright. The operator was transported via ambulance to South Lincoln Medical Center for injuries he received from the incident.

The employee was leaving Mine Medical on her way to the warehouse. The employee was stepping off of the last step of four steps when her knee gave out. She tried catching herself unsuccessfully and fell to the ground contacting her face onto the ground. This caused a chipped tooth, facial abrasions and contusion on her left knee.

While rappelling during a Mine Rescue Training, the employees left knee twisted when they contacted the ground. The employee did not seek any medical attention for several weeks. The ACL was torn and required surgery on 9/16/15. Work activities were not restricted until that date.

An employee was working on a haul truck when dirt and rust debris fell making contact with their face. Some of the debris went between the safety glasses and face getting into the employees eye. The employee was transported to the hospital where a pysician was able to remove the material and an antibiotic was prescribed for the eye.

An employee felt a sharp pain in his right knee while walking up the stairs of a shovel. The employee did not miss any work or have any restrictions until 6/17/15 when he underwent surgery to repair his knee.

An employee reported on 3/23/15 that his shoulder had been hurting him for a couple months. On 3/27/15 he saw a physician. An MRI was taken. It was determined that years of repetitive motion had worn out the shoulder and it would require surgery.

While EE was putting cutting edges on a grader the cutting edge fell off the magnet at approx. 3 feet high and struck the EE on the left foot. The cutting edge landed on the back side of the EEs steel toe boot causing a fracture in his left big toe and required stitches to his left big toe.

Employee was moving a metal plate and felt burning in his groin area. Initial treatment was ice and ibuprofen. Employee said he felt ok. Days later the employee had a doctor examination and it was determined a hernia that needed fixed. Surgery was done on 12/12/14.

An employee stepped in a parking ditch causing his knee to pop and causing pain. The employee was seen by an orthopedic specialist and an MRI was done on 10/17/14. This was the soonest he could be seen. He was released back to work on restricted duty on 10/17/14.

An employee was pulling on a rail switch when they felt a burning sensation in their back. The employee woke up the next morning with numbness extending down the left leg. After the investigation and an MRI, it was determined that the employee had an acute herniation of the L3 and L4 vertebrae. This information was made available to this office on 10/14/14.

An employee was experiencing shoulder pain in his right shoulder but wasn't sure if he hurt it at work. He reported this pain on 5/8/14. A surgery was scheduled on 8/6/14. After the surgery, the surgeon said there was no evidence of any one injury. The pain was a result of years of repetitive motion.

An employee was descending a platform ladder and slipped on the third step down causing him to twist his ankle. The employee resumed work without informing his supervisor of the incident. The employee had the ankle checked the next day and X-rays revealed that it was broken.

As employees were arriving for shift the smell of fire was noticed in the shop area. Employees then investigated to discover smoke on the hill north of the maintenance shop. A grass fire was identified and reported to the shift supervisor who then took action to notify the local fire district to extinguish the fire. No injuries or damage to structure occurred.

An employee lifted a chock block to place it back on a fuel truck. This action caused him pain in his right arm. No medical treatment other than ibuprofen was given. The employee stated that the pain never went away and it was looked at by a doctor on 8/5/14. At this time the employee was given work restrictions and surgery was scheduled. Surgery was done on 8/13/14.

The employee stepped onto the deck of an overburden drill that was wet due to rain and slipped. This caused him to twist his knee. The knee required surgery 20 days later.

While attempting to close the hopper door on a train car, one of the employees feet slipped on the snowy ground causing his little finger to get caught in the hoppers locking mechanism. This caused the bone in the tip of the finger to be broken and caused a laceration requiring two stitches.

The employee was bent over pounding stakes for a drill pattern. The employee felt pain in his lower back that radiated down his right leg. The employee was transported to the local clinic and the medical provider prescribed a prescription (muscle relaxer) which caused the employee to miss his next shift.

The employee was using a hydraulic ram to remove the rear wheels on Lowboy. As the ram pressured up, it slipped and smashed the employees left thumb between the ram and the inside of the wheel. The employee was transported to the hospital where three stitches were required to close a laceration on the thumb.

The employee was working on a booster pump. While pulling on a six inch pipe trying to get it into place, the employee felt a pop and tearing sensation in his left shoulder. This shoulder had been injured previously in 2010 that required surgery.

Employee was removing an apron cylinder from a scraper with the overhead crane. The cylinder became stuck, employee wiggled the cylinder loose. When the cylinder came loose his right middle finger was pinched between cylinder and machine. Employee was taken to the Medical Center, finger was x-rayed and showed finger was broken.

The employee was walking on the deck of a haul truck with both of her hands full when she tripped and fell. When the employee fell, her head came into contact with the coolant reservoir tank causing a slight fracture to the employee's forehead.
