The EE had just positioned the rail car in place for material loading. EE locked the brakes up on the rail car and stepped down on the ground to walk back to the rail car loading station, and stepped on the south rail of the rail road track. The rail was slick from the rain and EE's foot slid off the rail sideways causing EE to roll ankle over to the side.

Employee was coming down the shaker ladder and as they got to the second to the bottom step their right boot slipped off the step and they slid down, as they hit the ground their right knee twisted.

EE was washing under the feed hopper, a baseball sized rock dislodged, striking EE's hardhat causing a cut on EE's head. Treatment was offered and refused, refusal waiver was signed. After work, EE sought treatment, receiving one staple in EE's head. No documentation was received. EE was on vacation for the next three days.

Employee was releasing brakes on rail car while standing on the rail car plat form. He tripped as he turned around and fell. He injured his left shoulder.

The employee climbed up the front of the shaker screen where there is no safe access and was cleaning excess material off of the screen chute. When he finished he was climbing down and snagged his glove on the frame of the concrete conveyor causing him to over extend his arm.

Lack of communication. Crusher was locked out, screening plant running. Victim apparently did not hear or heed warning from crusher operator, walked under crusher. Co-worker was removing rock threw off crusher struck EE as he emerged from under crusher. He was wearing hard hat.

Installing belt on crusher pulled back no witnesses notified on 7/26 was going to be checked out at hospital doctor said pulled muscles gave paid meds. sent medworks 7/31/12 sent back with RTW slip no treatment needed. Did not report at that time appeared treatment was diagnostic and preventive. Called in 8/16/12 in pain did not come to work.

I send 2 employees to clear a area of logs. I told them not to lift anything. Send 2 one to hook chain one to unhook chain Cat man was up then one of the employees lifted a branch.
