The employee is unsure of what happened; He was working at a wash plant operating the excavator when he noticed he had pain in the groin area. He went to the doctor after his shift was over

Mechanic was performing repairs to an excavator when a wrench slipped off of a bolt, causing his elbow, shoulder, and neck to strike the components on the excavator. Caused pulled muscles, contusions and strains to the affected areas.

Miner was had been involved in his duties, which was loading used tires on a trailer and pickup. The miner suffered a heart attack when he was sitting in the cab of the pickup. The coworkers acted quickly with emergency first aid and drove him in the pickup to the emergency room (quicker than ambulance). Then miner was pronounced dead approximately 1.5 hours later.

EE was descending fixed ladder on lube truck when he slipped & fell to ground injuring Lt knee. EE stated knee was sore but didn't think it'd require med. attn. In Nov '14 EE went to dr as knee was still bothering him. Dr found small tear in meniscus, suggested surgery, scheduled for X-mas brk. Dr originally thought no restrictions needed but now suggests restr. duty till 2/14/15.

Employee stepped down off the steps of the lube truck, he did not notice a frozen chunk under his right foot, when applying full weight on his right foot, the frozen chunk rolled causing his ankle to roll also. A quick look around the landing area before descending the steps would have identified the frozen chunk and could have prevented the accident.
