Employee was bar scaling in the back of a room when a piece of rock from behind where he was working came down and struck him on the backside of his head. His hard hat did not cover this portion of his head, it struck him about two inches above the hairline on his neck. Resulting in a laceration to the back of his head.

Employee went to mount tongue of trailer that pump was mounted on, to the ball on the maintenance truck. Truck was not aligned with the tongue so employee went to move the tongue over. Employee moved the tongue too far over causing the tongue on the trailer to slip off the blocks and smash his finger under the tongue.
