Employee was helping another to reposition a gear box. While lifting wrist popped. Employee said that they did not hit or smash the wrist and did not understand how it happened or why. Said the wrist has been broken before and has a pin in there. Went to ER and is sprained.

Employee was grinding, with a hand-held grinder, some metal brackets to hang cable on, when metal shavings flew around safety glasses and into left eye. No symptoms at first until after work hours.

Employee was lifting 5 gallon Hydraulic oil buckets to fill a hyd reservoir on a loader. While lifting EE noticed a burning sensation on lower right abdomen. EE has had previous surgery to same area 10 yrs prior. Is waiting an appointment with dr.

Caneyville Crushed Stone LLC · Fall to the walkway or working surface

Employee was attempting to tighten a bolt on a Cat 225 Excavator Hoe Ram. Using a wrench to tighten the bolt, the wrench slipped off the bolt causing the employee to lose his balance and fall onto his behind. The employee is claiming that the incident happened on 10-9-2015, but did not report to safety mgr until 10-20-15.
