Employee was operating a D9 dozer pushing off the overburden dump. When backing up, the dozer ran over a large rock which caused the back of the dozer to rise and then fall over the large rock.
Employee was welding grizzly rack for structural repairs and received welding flash to right eye.
Employee was working routine work and it was a very windy day. Dust/dirt particles blew behind EE's safety glasses into EE's eye.
Employee was walking and tripped on frozen ground from tire travel tracks. Employee failed to have proper awareness of surroundings

Employee using grinder broke thumb. Should have used vice to hold metal prior to grinding.
Employee not following procedure climbed 6' ladder that was not level, employee was not wearing PPE gloves, the ladder tipped and employee cut finger.
Employee was lifting hammer lift assembly without help. Equipment too heavy for one person. Employee stated was angry with co-worker so didn't follow procedure. The hammer assembly slipped hit right big toe. Employee has language barrier. EE didn't inform us of the accident until investigation was launched on 8-13-19.
Employee got finger caught between screen and guard.
Employee was trying to dislodge a rock from the primary crusher with a 30lb jack hammer when a piece of rock struck EE in left wrist and arm knocking EE backwards.
EE noticed winch cable had slack and was hanging down the side of dock. EE grabbed a pole to try to grab cable and move it to proper side of gavel. The winch cable was activated causing the cable to jump the gavel striking EE on the outside of left knee.
Employee was walking near river bank feeling with foot to determine edge of bank. Once employee felt employee located it employee knelt down to feel with hand the ground gave way and employee fell about 10 feet.
Began work and was fueling up the excavator. When fueling was complete stepped off the excavator onto a step, slipped and fell to the ground on EE's right elbow.
It was stated that the accident happened on 12/29/2015 but not reported until 1/8/2016. Was hooking water hose to fill up the water truck and tripped over wooden blocks and lost his balance. Says he aggravated his left shoulder.
Employee was washing out from under the crusher platform using a water hose and spray nozzle to clean out build-up that had accumulated over the last week. While doing so the water hose burst causing the hose to whip and splashed mud and debris in his face and right eye.
Employee was engaged in loading barge when dropped a rope into river. He became very angry with himself, hit building with hand which resulted in broken bone in hand.
