The heating unit in the office trailer caught fire resulting in the roof of the trailer burning. The structure did not burn completely down but, it did sustain serious damage.

Employee was replacing a pump on the property. Employee unhooked the chain, used to hoist the pump, from the 325 Caterpillar Excavator when a hydraulic hose blew causing the bucket to crash onto the pump and also hitting the employee's head, neck and back area.

Employee was grinding a groove in a head pulley when the grinder kicked hitting the employee in the mouth. The employee sustained and injury to his lip resulting in the employee receiving stitches.

The employee was instructed to grease the belt lines at the prep plant. While greasing the lines, the employee slipped. He was able to catch himself before falling but twisted his back while doing so.

The employee was assisting a co-worker in removing a motor from the prep plant using a chain hoist. The chain slipped through the chain fall resulting in the hoist jerking the employee due to the weight of the motor, causing injury to the back of the employee.

He was spray painting steel and stopped because the sprayer was cut off. He came down the stairways holding on to the handrails. He got to the bottom floor he went down and was lying on the floor. When asked if he wanted to go to the hospital, he shook his head "no". He was taken to the local ER even though he shook his head "no". He was spray painting and overcome with heat.
