While changing a belt on a roller, employee was checking alignment of belt by pulling through roller, momentum pulled fingers of right hand in contact with roller causing injury.
Employee was draining a valve, was still liquid inside, when opened splashed on employee.

Employee was running conduit when employee bumped the junction box, disturbing wasps, employee was stung on L wrist.
employee was closing bottom of primary valve with turn bar, began spraying caustic, employee jumped to dodge spray, dropped turn bar on L foot, all PPE in place, was treated onsite, cleaned, ice wrapped, ibuprofen, after a week 09/12/2019, bruising and swelling diminished, still painful, trip to occ med shows small fracture of L great toe
Employee was with spreading hot Alumina to prep it for vacuuming. EE was using a 3" straight edge rake, to pull the material towards EE to spread it out. As EE did this, the material came towards EE like a wave and "blew" at EE causing it to go at EE's leg and run down into EE's boot, causing 2nd and 3rd degree burns to the top of EE's right foot to just above EE's ankle.
employee was walking on a ramp, there was a small amount of bauxite on ramp, EE stepped on it causing EE's knee to twist, it was not determined that EE required surgery until 9 October 2019. This employee did not report injury until 8/8/2019, EE has worked all but one day (4 Oct 19). EE is currently working full capacity. Surgery is not scheduled as of yet.
Employee noticed radiator cap on excavator was leaking, tried to tighten with right foot, when employee did cap blew off and hot fluid sprayed on and into boot, there was a hole in the boot, dorsal aspect of employee's foot burned
Employee removed PPE to wash off face and used a hose that was laying in sludge that had caustic in it.
MOVING HOIST BASKET FROM ONE LOCATION TO ANOTHER, HANDLE CAME DOWN COMPRESSING DISTAL PORTION MIDDLE FINGER OF RIGHT HAND CAUSING APPROX 1 1/2 INCH LACERATION AND COMPOUND FRACTURE TO MOST DISTAL PHALANGE
WALKING ON PLATFORM, PIECE OF GRATING MISSING, STEPPED INTO OPENING, STRIKING LEFT KNEE AND LOWER LEFT LEG ON GRATE.
Employee was wearing PPE (goggles & hard hat), opened screen box to drain liquid, some substance still inside under pressure was ejected onto employee, getting into their eyes, eyewash begun immediately.
Employee was working on number 1 tray in unit 345 when caustic sprayed into employees eyes. Full proper PPE was on at the time of the incident.
Employee was stepping onto the pedal of a bicycle when foot slipped causing the bike and self to fall.
Employee was in break room, turned to pick up drink can, turned twisted knee and felt a "pop". Complained of difficulty bearing weight on R leg. Evaluated in medical went home. Returned next day and continued to work regular schedule, not missing any work. Was seen by MD 09/10/2018 and had MRI 09/24/2018. Returned to work from MD visit wearing brace. Last day of work 09/30/2018.
Employee was moving a 2 inch diameter shaft, 16 foot long and fell on left hand.
OP4 bent over to lift a six inch hose so it could be relocated for the portable sump pump. When EE attempted to resume an upright position, EE felt a sharp stinging pain in the middle of EE's lower back.
Employee was late reporting a job-related injury (6/11/17) to plant nurse. On the morning of 6/18/2017 employee reported infected callus to nurse. Upon observation, caustic burn to outside of right big toe noted.
Employing was hydro-blast under Precipitator #6. Positioning platform shifted due to back-thrust on blasting shotgun. Employee lost footing and struck elbow on guardrail.
Operator was closing 14" vapor valve on outside of heater deck on west side of 340 area during a routine heater swap. EE felt a strain in left groin area. Nurse came to plant site and a decision was made to bring ee to the emergency room to get further evaluated.
OP3 was in area 335 turning a valve on #8 press when they felt a pain in the right abdomen that radiated down into groin. Employee seen by doctor for evaluation.
Employee was going up in the elevator to third level of production unit to report to employee's control room. While elevator was moving upward, it stopped unexpectedly, trapping employee in elevator.
GRM was in field maintenance shop reaching for a jack stand in a squatting position,& when ee attempted to resume a standing position, ee felt a muscle strain in lower back on the left side. EE reported back strain to nurse, and was later brought to Dr. for an x-ray and evaluation. Prescription medications ordered and out of work for four days.
Employee was in the process of pre-operational testing an angle grinder. After several attempts working the start switch, EE did not anticipate the initial kick of the tool as it came on. The tool was in the left hand and kicked to the right. The cut off blade on the tool struck the index finger of the right hand. Causing a laceration requiring sutures.
White side supervisor was in the process of switching fill pumps when the surge tank overflowed. The sump pump could not keep up with the flow. The liquor level rose quickly and entered EE's boots resulting in thermal and caustic burns to both lower extremities. Transported by RN to Baton Rouge General ER and hospitalized on Burn Unit.
MWT was working in maintenance shop. While seated on a stool ee reached forward to retrieve a wrench with left hand, and ee felt a pull in low back. EE was transported by ambulance to St. Elizabeth's ER where ee was treated and released to home later that evening.
Driver (Sun Coast employee) fell from 2' height when handrail on a tank platform/walkway driver leaned on broke. Driver fell to the ground and broke upper arm. The handrail was constructed of wood and was in a weathered condition.
Employee leaned on handrail of walkway and it broke, causing EE to fall several feet to the ground and break EE's arm. Accident originally reported to OSHA under SER 56522 before it was know this was a MSHA-regulated location. Reporting now due to oversight of reporting to MSHA at the time.
Operator was performing task of changing out condensate filters & was in the process of putting the condensate pot back into service. When operator started opening the condensate valve to fill the pot, hot condensate started spraying out from the pot gasket & made contact with right abdomen, right hip, both thighs & buttocks. Initial diagnosis was 2nd degree burns to all areas.
Report only: CAH reported to Gulf Coast for return to work after being off for personal reasons. CAH informed Gulf Coast staff that EE's injury was related to work activity. No specific event was identified that led to the injury. CAH remained under the care of EE's personal physician until EE returned to work on 10/12/2016.
GRM in Area 340 went to install a valve on a vacuum flash pump when left foot slipped resulting in EE tearing calf muscle. EE was monitored daily for one week, then was seen every other day for the second week. In the third week, on 10/17 EE had swelling in the lower leg & saw orthopaedic doctor and was diagnosed with a tear. Physical therapy was ordered twice weekly.
Employee was in the process of opening the discharge to the #1 vacuum pump. When employee struck the valve with the maul the valve swung around and smashed the middle finger between the valve handle and the maul handle resulting in a laceration to the (L) 3rd finger.
Employee was operating a bobcat to remove scales from the clear pump pad area. The bobcat got stuck in the relatively narrow passage between the #2 and #3 clear liquor pumps. While attempting to exit the passage the bucket of the bobcat contacted a drain valve on the discharge line of the #3 clear pump. The impact broke off the drain valve from the line.
Employee was walking between the ditch and the ESP MCC when employee slipped in the wet ESP dust and twisted ankle. Medical evaluation resulted in prescription medication and a (R) fibula fx.
Employee stated they had pain in back while operating mobile equipment, hit a bump in roadway and the seat suspension wasn't working properly which resulted in discomfort to the back.
Employee was in Area 345 inspecting #31 precip airlift when drop of caustic dripped and landed on employee forehead. When EE tilted EE's head forward, the caustic ran down inside the top of EE's goggles into EE's left eye. Medical evaluation resulted in prescription medications.
GRM was in area 345 working on #2 secondary cooler when EE attempted to use a wedge & pry bar to get the cooler off the studs. When cooler came free, it lifted slightly and upon coming back down on the studs it smashed right index finger. Brought to local ER afternoon of 12/20/15 & was given prescription meds, then seen by ortho specialist on 12/21/15. Had surgery on 12/22/15.
EE used improper rigging technique when rigging a lever hoist with 2 sling chokers spread & connected to hook at lever. Tightened up on load choker slipped to safety latch causing it to break. Block & lever sprung & hit ee in mouth leaving a minor laceration & chipping 2 teeth. EE was prescribed an antibiotic to prevent infection inside mouth.
Employee was hydro-blasting some piping using a flex lance with no anti-withdrawal device on the flex lance. The flex lance came out of the pipe and struck the employee in the right upper thigh causing a water laceration to the upper thigh approximately half an inch long.
GRM was working on ground floor of area 350 cold end. He was on a clean-up project following a turnaround when a pan filter support roller fell down a tugger well and struck him on the right foot. X-ray at Gulf Coast Occ Med revealed a distal tuft fracture to the big toe on the right foot. Returned to work regular duty with no restrictions.
GRM was at rod mill #2 washing the screen on the north end. When he got splashed with caustic water, he immediately went to eyewash to rinse, When he removed his goggles, caustic dripped in the corner of his right eye. Washed in medical dept by RN for 45 minutes and returned to work. Seen by Dr. on morning of 3/27/15 for redness & swelling. Prescription medication ordered.
Supervisor was walking in Area 345 when he tripped over slightly uneven grating falling forward & catching himself with his hands. Received laceration to the left palm as a result of fall.
Employee had chemical contact to right eye.
While working, employee was opening clam shell on reclaim chute. He got his left small finger smashed between the clam shell handle and the flop gate handle resulting in left little finger getting smashed with small laceration to the tip. X-ray revealed finger tip fracture.
Store clerk was in yard behind stores stacking pallets, when he dropped a pallet and it smashed left ring finger. Employee was not wearing his proper PPE at the time of the accident.
Employee was removing filter pan side shield pulling upward, when side shield broke free the employee hand moving in an upward motion struck a 1/2 pipe nipple; causing a puncture wound. Note:Employee received stitches and pain medication.
CAH was in calcination area shutting down #2 injection pump from caustic cleaning. When shut down, he opened the drain, and the drain on the suction sprayed the left lower leg. Rinsed underneath #1 hytank for 5 minutes before going to dispensary for 20 minute shower. Evaluated by RN and seen by Dr. for pain medication. Returned to work on 09/30/14.
Employee was trying to unplug #3 filter feed hopper with a maul. While swinging the maul, a piece of foreign body flaked off, making contact with his right eye. Emp was not wearing the correct PPE for this area. He should have been wearing goggles, but instead was wearing safety glasses.
