Showing all 9Employee stated fork fell off of skid steer. When EE tried to place it back on, EE grabbed it in two handed grip and felt the muscle in left arm roll up.
The employee stepped off the bottom stair onto uneven ground and rolled left ankle. The employee could hear an audible pop and noticed significant swelling, the employee was able to drive themselves to clinic. An x-ray concluded no fracture and employee refused prescription ibuprofen, and was given a walking boot in case needed.
Loader operator was dumping loads into haul truck however, employee felt that loader operator was hitting the side of the haul truck with bucket. Employee maintained that their back was hurting due to the impact. Resultant injury was mild muscle irritation with no long lasting problems.
Operator had gotten out of the piece of equipment and started walking down the ramp and rolled an ankle.
The miner was removing a rock stuck in the feeder of the primary crusher when it became dislodged and foot was wedged in the hood of the crusher.

EE was performing irregular maintenance on a piece of equipment. This piece of equipment was properly locked out and tagged out. EE reached EE's hand inside the machine to adjust a part when the machine shifted and crushed EE's hand.
The employee dropped a tool into the jaw crusher while observing a jammed rock, then jumped off of the elevated catwalk to turn the feeder power off. EE reported twisting ankle to the supervisor and was sent home to ice ankle and return the next day.
