Showing all 5Employee was in # 2 finish mill replacing shell liners. Employee was guiding the liner in place & lost footing on a corner of another liner that was already installed. The momentum moving forward caused the liner to drop and the right ring finger to be smashed between the mill shell and the liner. The supervisor did receive stitches to right ring finger.
Employee was using a wrench to loosen bolts on a valve on top of the cement silos. As employee was pushing down to loosen the bolt felt a pain in right shoulder. There was no damage to the shoulder but was issued muscle relaxers.
Employee was chipping the slag off a weld with a hammer. A piece of slag flew into left eye. The employee was wearing prescription safety glasses with side shields. The employee was working over the weld and the slag flew up and underneath the glasses. There was no damage to the eye. Antibiotic eye drops were prescribed for prevention.
Employee was using a 4" hand grinder and the grinder kicked back while in use and the employee was cut on the left side of the neck.
