Showing all 7Employee was trying to free blockage of rock/material in a tail pulley using a pry bar. The pry bar slipped and the employee fractured EE's right ring finger. Equipment was de-energized and locked out when employee performed this task.
Employee dismounted the basket of the ANFO Loader and stepped on a chunk of limestone, twisting EE's left ankle.
Employee states was working with a come along when employee felt a pop in shoulder. The employee did not report the accident to supervisor. When the supervisor became aware of the allegation the employee was taken to the clinic for a medical evaluation and released with no prescription medication, told to rest shoulder and return for a follow up exam in a few days.
Employee stated that the compressor on the drill overheated, as a precaution, the employee exited the machine and jumped over a berm.
Employee stated was changing a hose on the roof bolter, pulling on the hose, and had discomfort in the right arm. Employee started physical therapy on Nov 4 to make injury reportable.
Employee stated EE pinched finger between the boom and the basket when the stop block broke off and allowed the boom to lower more than typical.
