Employee experienced a diabetic emergency after being found to on the ground, unconscious and unresponsive. Employee has a known history of Type-1 diabetes. Local mine EMS was notified and employee was prepped for transport to a local hospital where employee was evaluated, treated, and released without restriction.
Climbing down ladder felt pop in right knee
Employee was loading a job box in the back of a service truck, job box slipped off forklift smashing left thumb.
Moving wooden pallet in shop caused lower back strain.
Employee was helping to get a blade unstuck and when it slid in mud and jarred their back.
Employee was walking and twisted ankle.
Employee was exiting driver side of service truck and tripped on broken concrete and rolled ankle. Initial doctor's requested imaging on injured ankle. Results came back 6/25/25 and the employee was put on restricted duty.
Employee was Pulling drip line on pad 4 rehandle. Coupler gave out causing employee to fall backwards. During fall employee twisted left knee and was not able to put full weight on that leg after fall.
Employee was using a pin from a clevis as a hammer to loosen tow rope from clevis. Employee went to an ER and received four stitches in left middle finger and medical glue on left ring finger. X-rays showed no fractures.
Stepped off of a Track Dozer while the dozer was on a tow haul trailer and missed stepping on the trailer and falling to the ground
Exited the cab of a Hydraulic Shovel to use the restroom and the jacket of the employee snagged on the door and slammed on the employees left middle finger. Employee was seen at the ER and had X-Ray imaging done on the affected finger. X-Ray showed a hair line fracture on the affected finger. Employee received a return to work no restriction on next scheduled shift (11-25-2024)
Employee was unloading tools from tool box and putting them on a tool cart. Employee felt discomfort in the lower abdomen while pushing tool cart. Employee notified supervisor.
Employee was walking slope on a leach pad and plumbing solution pipes. While walking on the slope the material was wet and caused the employee to slide and then tumble down the slope. The employee had a strained neck and right ankle due to the incident. The Doctor recommended that the employee take three days off of work.
Employee was opening a valve, and the valve lever broke causing the employee to fall to the ground. Employee was seen at Aspen quick care in Spring Creek, NV. X-Rays revealed that the employee had a fractured coccyx. the employee was released to full duty and will return to work on next scheduled shift, June 15th.
A safe rescue line attached to the back of an employee standing on the ground got entangled on a handrail of a Bulk truck while pulling through an ammonium nitrate silo. With the line being tangled, it got tight and jerked the employee off their feet and into the ground causing injuries to left shoulder and right bicep.
Employee was dismounting a track dozer and tripped forward and felt the knee pop and was taken to Hospital in Ely, NV. Xray performed with negative findings and released to full duty with. Followed up with Ortho and MRI was completed on 04/23/2024. MRI results read on 04/24/2024 with the ACL and meniscus was damaged. Released to restricted duty on.
Progression of hearing loss at high frequencies bilaterally noise induced.
Employee was driving a boom crane out of mine traveling on the 8060 area of mine when they lost traction and the crane began to slide/roll back down the hill until it ran into the left-hand berm/hillside and tipped onto it right side. The employee was wearing a seatbelt but sustained a concussion, compression fracture in lower back, and multiple contusions.
Bilateral moderate high frequency sensorineural hearing loss.
Bilateral moderate-severe high frequency sensorineural hearing loss. STS bilaterally.

Walking around equipment and slipped on frozen ground
Employee was cleaning the welding area of the deployed geomembrane liner with their back to 2 other crew members who were rolling up a scrap piece of geomembrane material. The rolled up scrap geomembrane got away from the two crew members and contacted cleaning employee on back of ankle causing employee to fall to their knees.

Employee was tightening bolts when right index finger got smashed by wrench resulting in a fracture of the right distal phalanx.
Technician began experiencing symptoms of heat illness while working.
While removing a filter the wrench slipped off and struck employee on the lower lip causing a laceration resulting in the employee receiving 3 stitches
Employee initially reported that dirt/debris had gotten into EE's eyes during windy day. First aid was refused on site and C1 was completed. On 01/04/2022 employee went to optometrist and found that a microscopic object was lodged in left cornea. Object was removed.
The operator of the excavator hit the left rear corner of the 605 haul truck with EE's bucket while swinging to dump the bucket load into the truck.
A haul truck operator was traveling at an excessive speed down a ramp when the haul truck encountered a containment channel. The truck hit the channel and bounced the operator in their seat, which caused a lumbar fracture in the lower back.

Operator was dismounting water truck when EE missed the last step and sprained knee when EE's foot hit the ground hard.

While walking over uneven ground and carrying a heavy box the employee strained the left side of abdominal wall.
While walking down hill to a drill site being constructed employee twisted right ankle.
While loading a Gehl forklift onto a flat top trailer, the operator attempted to better center the forklift. In doing so EE got to far to the right side and the right side of the forklift slid off the trailer edge. The operator sustained a cut on right forearm requiring 21 stitches.
While employee was removing left rear brake rotor it fell and pinched left middle finger.
Employee placed right hand on door frame of track drill. Wind blew door shut injuring the thumb.
