Showing all 4Employee was trying to reach a 1/2 ball valve to open it and it was slightly out of reach. EE jumped to get a hold of the handle and when EE landed on EE's foot EE turned EE's left ankle.
EE was inside plant working & placed left hand on hanger frame & backguard f/forklift frame caught tip of middle finger pinching it severely. It was a severe break of tip of bone of finger as xray showed. EE had surgery on 8/5/21 and has been released by doctor to return to work on 8/10/21
We had a cut piece of metal to make lifting lug, didn't get sharp edges ground down. Was going to take it loose from pump cover after it was removed and the cover wasn't secured and started falling, and the employee grabbed the metal and stuck sharp point in hand and it jerked EE's hand and cut it.
