Employee was bolting top and dropped wrench. EE stepped down off of the platform to get the wrench and slipped on the muddy bottom. EE's knee went one way and EE's body went the other.

Employee was attempting to load a draw head from the rock duster up on top of the duster and when lifting the draw head said that EE felt and heard back pop. The draw head weighs between 100 and 150 lbs.

Attempted to roll a pump motor off mantrip when I bolt on top of motor broke causing employee to lose balance falling to the ground. Employee hit hard enough causing EE to chip tooth and jam neck and back.

EE climbed the ladder to get back in position to operate the loader. As EE was climbing, EE missed a step & fell forward which caused EE to reach out to the hand rail to catch self. When EE grabbed the handrail EE had pain in left shoulder & the pain has continued in left shoulder since this incident happened.

The employee was helping spot a rib bolt when a rock rolled out of the rib and struck EE behind the metatarsal guard of EE's boot. The rock punched a hole in the top of EE's foot that was too deep the clean, so the medical facility did not clean it right away. This prevented stitching of the wound.

EE was pulling excess curtain from the mine floor to advance curtain on bolt cycle in the face. A rock measuring approximately six inches wide by seven inches long by three inches thick dislodged from the mine roof and struck ee on left shoulder. EE finished shift but went to the emergency room later in the evening with complaints of shoulder pain.

Employee was in the process of cutting belt rubber with a knife and while cutting the belt the knife slipped, causing them to cut their left thumb. This resulted in a laceration that required 5 stitches to close the wound.

Employee was on knees cutting plastic bands off of a new joint of miner waterline for replacement. EE lost balance causing EE's weight to shift. As EE tried to regain balance, EE cut left thumb.

2 employees were supplying 2 section. The outby motorman told the inby motorman to drop down 10' to be lined up to unload. When the inby motorman applied the brakes, the trip started sliding downgrade toward the mainline switch. The inby motorman stated EE was ejected when the motor derailed at the mainline switch.

Employee was dropping cables in preparation for a power move. As EE dropped the cable, EE's left ring finger came in contact with a rib support (pizza pan) causing a laceration to the side of EE's left ring finger. EE did not receive any stitches, but was referred to a had specialist for further examination/ treatment. EE was allowed to return to work on restricted duty.

Employee was retrieving CO cable from 2-A belt head for installation on the 2 section belt move. EE placed right foot on the fresh water pipe to gain height to reach the cable. EE's foot slipped and caused right knee to buckle. EE sought medical attention after the shift and was placed on restricted duty.

Employee was loading roof bolts onto the Roof Bolter. While doing so, a piece of mine roof approximately 8" x 8" x 2" fell and struck EE on the right middle finger causing a laceration that required part of the fingernail to have to be removed and 2 stitches to close the wound. The mining height is 8 1/2 feet in this area.

The employee was constructing a stopping at 59cc #1 Section between #7 and #8 entries. The area was 9' 3" high and 18' 8" wide. EE was standing on a ladder to place another block on the stopping. As EE placed the block, a section of rock measuring 24" x 13" x 1" fell out and struck EE on top of the right hand. The injury took 7 stitches to close.

As the Day Shift Pre-shift examiner entered the mine property conducting EE's examination (around 4:45am), EE noticed the #314 Dozer engulfed in flames. The machine had been parked for about 11/2 hours after the conclusion of Evening shift. There was no one else on the property at the time of the discovery of the fire. EE immediately reported the fire on the 1-800 number.

Employee stated when loader placed large rock in the bed of employee's haul truck, the rock rolled across and struck the off side of the bed. This caused employee to be jarred inside of the operators cab.

Employee was walking toward the section and slipped in a Scoop rut twisting right ankle. The mine floor was wet and muddy at the time of the accident. Employee returned to work the next day (6-1-18) with no restrictions. Employee returned to the Doctor on 6-8-18 and was put on restricted duty for 2 weeks beginning on 6-9-18.

An unplanned roof fall on the off side of the belt in a crosscut was discovered by the examiner during EE's examination. It did not impede travel or airflow. There were no injuries. The roof fall measured approximately 90'L x 20'W x 6-8'T.

Employee was riding on the back seat of the shuttle bus when the bus operator abruptly hit the brakes due to holes in the road. This caused the employee to be thrown out of the seat and strike the seat in front of EE. EE's left leg was caught under the seat in front of EE and EE's weight went over the seat. This incident became reportable on 3/27/2018.

Employee had just finished drilling a roof bolt hole in the #4 entry on the #1 section. The drill steel and chuck saver detached from the drill pot and stay in the hole as EE was lowering the drill pot. The chuck saver suddenly released from the drill steel and fell approximately 3 feet striking the employee on the left hand.

Employee was getting ready to drill a roof bolt hole in the mine roof. A piece of rock measuring approximately 1'L x 1'W x 3"T fell from the mine roof when the drill steel touched the mine roof. This resulted in a laceration to EE's right forearm and took 4 staples to close the wound.

Employee was attempting to use the quick release to let down a 10 ton Simplex Jack. The plunger on the jack quickly came down and caught the employee's left middle and ring fingers between the jack base and plunger causing lacerations to both fingers.

A fall of the mine roof was discovered at Brk 40 of the Johnson Fork Mains, the fall occurred from between the #4 and #5 entry over to the intersection of #6 entry. This fall measured approximately 120 ft Long X 10 ft Thick X 20 ft wide. This fall does not impair ventilation or impede travel in this area.

Injured employee and two other employees were changing the cable reel on a shuttle car. They were using a 3/4 ton chain ratchet to pull the cable reel out. Injured employee was using left hand to operate the chain ratchet and had right hand resting on the chain the chain ratchet was hanging on. The cable reel shifted causing the chain ratchet to slide down the chain.

Employee was restocking the supply tray with 6"x 6" roof bolt plates on the Fletcher Walk-Thru roof bolter ee was operating. EE was carrying 2 bundles of roof bolt plates to the supply tray. EE lost balance. This caused the 2 bundles of roof bolt plates to smash right index finger.

An unplanned roof was discovered in the #7 Entry on the #1 Section at approximately 8:50 a.m. The roof fall measured approximately 90 feet in length and was approximately 6 to 6 1/2 feet thick. It occurred approximately 25 feet outby the #23 crosscut to approximately 40 feet inby the #21 crosscut.

Employee was struck by a small piece of rock on the right foot. Employee suffered a contusion to the top of foot. Employee continued to work until 2/14 when employee went to the ER with a small swollen area. The employee is a diabetic and as a precaution the ER Doctor recommended restricted duty for 3 days. Employee returned to normal duties.

An unintentional roof fall was found by weekly examiner in the Johnson Fork Mains at spad no. 7478 in the no. 7 entry beginning at x-cut 34 (inby approx 1/2 way of the block) and extending to the 36 x-cut, 20 feet wide and above the anchorage of the bolts.

DURING THE WEEKLY EXAM THE FIRE BOSS FOUND AN UNINTENTIONAL ROOF FALL AT 34BK IN THE #5 ENTRY. THE FALL WAS 7 TO 8 FT THICK IN THE Intersection. this FALL WAS IN A AREA THAT IS NOT NORMAL TRAVELED AND DOES NOT AFFECT VENTILATION CONTROLS.
