Miner clocked in from lunch and was using their phone while walking. There was a wheel chock out in open space that led to them tripping and the miner fell. The fall then apparently caused a fracture to the proximal end of the radius bone in the left arm.

Miner was operating a CAT 12G grader on an incline, the engine stalled during operation. The miner removed seat belt and jumped off the piece of equipment. They landed on their feet but then slipped onto their buttocks thereby injuring their back.
Employee was cutting belting used to guard a conveyor with a box cutter knife. The knife slipped and EE lacerated EE's right hand.
