On May 13, 2026, at 1:15pm, an employee ignored an operator's warning, removed a guard, and reached into a moving conveyor to apply dressing. EE's arm caught, pulling EEs forearm into the headroller & pillow block bearing. Bolt in bearing caused a deep laceration. First aid was given & EMS responded. The worker was hospitalized for surgical repair of the laceration & a muscle tear.

Truck operator was clearing materials off from the tail gate with rt hand when the latch mechanism pulled the gate inward pinching the tips of operator's fingers & glove. Operator was taken to ER where they sutured the distal tips of right middle & ring fingers back in place. On Sep 27 a hand specialist determined that right middle & ring fingers distal needs to be amputated.

Employee was attempting to clear material from chute by hitting it on the side with a sledgehammer. The sledgehammer was heavy, so employee put right hand up on the handle to control it. Employee pinched right middle finger between the sledgehammer and the bin. Employee was brought to the hospital and found to have fracture of the distal aspect of the finger.
