Showing all 4Employee was attempting to start a thermal fluid heater. After multiple attempts the thermal fluid heater fired causing the control panel to fly open hitting the operator in the face causing a hairline fracture to the nose and five stitches.
After receiving the C-4 on 3/13/18 and completing an investigation it was revealed that the employee was throwing pipe down a shoot when one of the pieces of pipe got lodged on a hard spot and the force sent a shock up the employee's arm causing the strain
After receiving medical paperwork on 1/24/17 it was found that employee slipped and fell due to ice on the ground causing a contusion and pain in low back.
