Employee suffered foreign body in their eye when turning on a shop fan. The fan stirred up dust in the shop. Employee was wearing safety glasses. First aid was administered(eye wash) but EE still felt something in the eye and requested medical attention. Employee was given prescription eye drops and returned to work.

An employee was walking in the chemical room to turn off a water hose being used to fill a tank when the employee slipped and fell on the concrete floor injuring back. The employee was diagnosed with nondisplaced left transverse process fractures at L3 and L4 vertebrae.

On 5/23/22 a maintenance employee was greasing trunnions on a sand dryer when EE felt something in EE's right eye. On 5/25/22 the eye continued to feel irritated, so EE sought medical treatment and a small piece of metal was removed from EE's eye.

An employee was trimming a dewatering screen panel with a razor knife when EE cut the tip of right index finger causing a laceration that required 5 sutures to repair.

Injured employee was pulling a water hose while cleaning up an area at the wet plant. While walking EE stepped on a sand pile causing EE to trip injuring EE's right knee and back. Employee was diagnosed with a sprained right knee and back and was given work restrictions.

A heavy equipment operator was dismounting an excavator at the end of the shift. The bottom access step was covered in mud and caused the employee to slip and land awkwardly injuring left knee. On 11/5/2019 the employee was diagnosed with a sprained MCL and placed on restricted duty as a result of the original injury.

EE was walking across even ground and went to step up onto the truck scale (approx 12-18"). As EE began to lift self onto the platform, EE felt a pop in EE's left knee. Employee was seen by medical personnel the following day and was diagnosed with an unknown internal derangement of the left knee and placed on restriction.

Employee was taking off a hatch from a Bucket Elevator, when EE accidentally dropped it on EE's Right Foot. Initially it was diagnosed as a contusion type injury. Upon follow up evaluation, the Physician diagnosed it as a fracture of the 5th digit little toe. This diagnosis was made on 7/10/2019.

Employee slipped on some sand, straining back and right hand. The Employee was medically evaluated by a physician, X-Rays proved negative findings, and the Employee was prescribed muscle relaxants.

Contractor (MSHA ID: B9304) was in the process of changing out a valve and mufflers due to sand build-up. EE was in the process of unscrewing the valve from the air supply line when pressure in the valve propelled it striking the contractor on the left side of EE's face directly in the brow line causing an approximate 1.5" laceration.

On 4/10 EE reported pain in right wrist. On 4/12 EE diagnosed with repetitive use disorder. On or about 7/14/19 disorder determined to be work related. Surgery on 7/26/19.

Employee was walking to raw feed area to collect a sample for quality control. While walking, employee tripped in mud and fell down. First aid was performed. Employee went to an occupational clinic on July 16, 2018, where employee was treated for a sprained right knee.

Plant operator stepped onto grating. The grating shifted, causing the operator to trip and fall. First aid was provided. Employee went to an occupational clinic on July 18, 2018, where ee was treated for a sprained right knee.

Diesel Exhaust Fluid splashed into miner's eye when removing the fueling nozzle. A medical review was performed on 8/22/18 that diagnosed the miner with chemical conjunctivitis and traumatic cataract of the left eye.

Employee was using a 4 lb hammer and hammer wrench to loosen bolts on a Rotex. While swinging the hammer it hit a junction box in close proximity and caused the hammer to impact their ring finger on the left hand. Impact caused open fracture with blow out laceration at the tip of the finger.
