Employee was attempting to manually slide a pallet (approx. 600 pound) inside a van to position for pickup by Lull/forklift. During this task, the IP experienced pain in their right bicep muscle.

Showing all 5Miner was exiting water truck. Miner was standing on top step of stairs and lost footing as miner turned to face stairs to go down. Miner fell approximately 30 inches, injuring left knee. Miner was taken to hospital by ambulance and diagnosed with dislocated knee and ruptured Patella tendon. Underwent surgery the next day. Released the following day.
Employee was shoveling walking area on side of barge. Employee stood up and cut lower back on protruding piece of damaged steel.
Feeder belt stopped, needed to be shoveled to restart. EE was shoveling belt when EE felt slight strain in upper right shoulder.
Crew was working to pour concrete for a new conveyor tunnel. Towards the end of the pour an employee noticed that concrete had run into EE's rubber work boots, came in contact with EE's skin and began to burn the skin.
