A loadout scale house operator smashed and caught left-hand ring finger as they were entering control room office. EE was taken to ER and followed up with an occupational doctor where they sustained a minor fracture at the tip of left-hand ring finger. The EE was not prescribed medication, nor was EE put on any restrictions and returned to work the same day.

Employee was cleaning out a screen inlet chute from clogged material that resulted in a laceration to the right ring finger. When the EE removed their hand, EE made contact with the top of their right ring finger and a sharp metal edge of the chute inner lining. The EE received six stitches and was allowed to return to work with restrictions.

EE reported on 9/16/22 but did not want medical treatment. On 10/4/22 EE stated EE did want to see Dr. for strain to Rt. shoulder from pulling silo slide gate pipe handle on bin number three. Slide gate was stuck closed, and the EE overexerted to open the gate. On 10/7/2022, Dr. did not prescribe medication and no restrictions but referred EE to PT.

Bagging Operator was helping a forklift operator align the lifting eyes on a super sack tote in the storage warehouse. The Employee's right ring finger was caught in between the forks and the building c-purlin resulting in a laceration requiring stitches.

Employee was working near Bucket Elevator 205 near the Screenhouse Plant when the Employee walked into a 2-inch angle iron support on an H Brace I-beam that was supporting a belt conveyor structure. The angle Iron was at approximant 5 foot high. The employee was wearing hard hat. Employee was prescribed muscle relaxants for a stiff neck.

A Maintenance Technician was tasked to remove a piece of angle iron that was stuck in the belt conveyor tail pulley of BC-101. The EE tried to roll the conveyor belt backwards to let the angle iron drop out of the tail pulley by rolling the motor drive belts at the head pulley. In doing so the EE caught three fingertips on left hand. The Employee was wearing gloves.

Employee was a passenger in a moving work truck, in the backseat and was reaching down to pick up a set of keys when employee overextended arm and felt a sharp pain in left shoulder.

Employee was using a vacuum hose attached to a vacuum truck to remove sand near a guarded tail pulley. The hose clogged and the employee was picking up the hose to dislodge the material to unclog it when the hose clog released and the hose fell to the ground, the employee then felt a sharp pain in EE's back.

Employee's right thumb was caught between a post hole driver and a ground rod while driving rod into ground. Employee received a laceration which required medical attention and stitches.
