Employee was directed to retrieve a part from another on-site location. A loader blocked the roadway, requiring travel around it on uneven terrain in a side-by-side. While driving, rough ground caused the vehicle to jerk toward the loader. The employee's left hand, holding the safety handle, was caught in a pinch point during impact, causing an open fracture to the left wrist area

A fall of ground occurred in the FED2352 level on May 13th, 2026. The fall of ground happened between shifts and was discovered by the ore control geologist team. Approximately 200 tons of material fell.

EE was installing 42" pipe when the wind blew into the pipe causing dust and debris to enter EE's left eye. Seen at eye doctor on 4/23, diagnosed with a foreign body in the eye which was removed and prescription eye drops given. Released to full duty on next shift.

EE was assembling core shed table. Putting bolt and nut on, had left hand on table holding bolt. While pulling down on the table, felt sharp pain in left side of chest. Seen at clinic, diagnosed with a pectoralis strain, released to light duty.

EE was repairing a defective windshield wiper unit. EE saw motor linkage was disconnected. While reconnecting the linkage, the wiper motor unexpectedly activated, pinching their left ring finger between the linkage and the ball. Seen at clinic, diagnosed with fractures to the left ring fingertip, and wound closed with sutures. Released to modified duty.

EE using hammer to loosen a bull hose from cam fitting. Other EE assisting and pulling on bull hose. Bull hose released unexpectedly while the EE was swinging hammer, and their thumb was pinched between the two pound hammer and the cam fitting. At ER, thumb nail was removed and one suture placed in nail bed. Small fracture at tip of thumb. EE released to restricted duty.

EE has been off work since 3/28/25. On 4/9/26 they contacted NGM to notify us they were diagnosed with an occupational disease, bronchiolitis. EE was seen by a doctor who confirmed the diagnosis of bronchiolitis. They kept the EE off work pending ongoing treatment.

Ee was disconnecting a fire hose, walking while looking down, turned EE's head and ran into the outrigger of a crane with EE's face. Fractured EE's front teeth. Seen at Any Day Dental for treatment starting on 4/7 and went back in on 4/8 to complete. Released to full duty 4/9.

Ee had climbed up into EE's haul truck and sat down. EE dropped a paper, pulled the arm rest up to reach the paper, when EE felt a pop in the right side of EE's right shoulder, then instant pain. Seen at ER, diagnosed with a right proximal humerus fracture. EE has been diagnosed with osteopenia which can lead to non-traumatic fractures.

Ee reported to NGM on 5/4/26 that on 4/3/26 ee was working 'in EE's columns' when a contractor and crew started hydro blasting above EE, causing debris to fall onto EE. EE reports that EE and a coworker yelled and went to the control room. Injury not reported to NGM until 5/4/26. No signs of injury to ee. Site cannot confirm incident occurred as reported by ee.

At approximately 10:35 PM on 4/1/26 smoke was observed inside the refinery via camera feed in the AARL Control Room. Security was notified. By 10:50 PM the affected cells were fully engulfed. Fire suppression was activated at 11:01 PM, and the fire was fully extinguished by 12:25 AM. No personnel were inside the refinery, and no injuries occurred during incident.

Ee reports steam cleaning on top of shovel when EE slipped an fell, landing on EE's right side and shoulder. Seen at ER where EE was diagnosed with rib and back contusions and shoulder strain. Seen at ortho on 4/9, diagnosed with AC joint separation. Referred for surgery.

EE was walking past an unoccupied jammer underground when they slipped and began to fall. EE struck out right hand to catch themselves, contacted a broken bolt sticking out of the rib, lacerating right palm. Seen at clinic, wound closed with sutures, and EE released to restricted duty. Site offering modified duty.

Operator on lunch saw EE's truck on pile of rocks. When contact was made, EE unresponsive, truck running, doors locked. Supervisor broke window to get to EE, EE remained unresponsive. CPR started and continued while EE transported to Hospital. This appears to be a possible cardiac event. Investigation ongoing.

Driver of crew change bus parked with front wheels in parking ditch. Upon leaving, they went forward, accelerated, causing the rear tires to enter the ditch at a higher speed, and the rear of the bus to lift. EE's seated in back of bus lifted from seats, and this EE landed awkwardly. EE was not wearing a seat belt. Seen at ER, released to restricted duty due to back strain.

The crew was pulling the liner down the slope and pulled a little to fast when the liner unraveled and caught speed, they tried to hold it with the forklift, it began to roll down the slope when the slope and the crew on the corners got out of the way, the employee in the middle tried to avoid by jumping and it caught their feet and they slipped down.

EE was dropping bolts off at headings underground. A bolt slipped out of their hands back onto the truck, and the bottom of the bolt hit them in the face, cutting their cheek. Seen at ER, wound closed with sutures, and the EE was released to full duty.

Ee operating 4.5in disc grinder fitted w/cutoff wheel to remove fender from flatbed trailer. Tool unexpectedly kicked back and the rotating wheel contacted the ee's left hand, cutting through the leather work glove, and cutting the pinky finger. Seen at ER, wound sutured, ee released to full duty.

Approximately 30 kg rock released from the face near the springline in active underground workings. Rock fell 5-8 feet and impacted the miner's arm against the workbasket handrail. Rock was partially restrained behind overlapping face mesh. Miner sustained arm injury.

EE was connecting a nip, lost footing, twisted and dislocated their patella. Seen at the ER and released to light duty. EE referred for surgical consult and on 3/11, NGM received confirmation from the surgeon that a tear in the knee was caused by the work-related incident.

Miners were attempting to remove an old pump discharge pipe. The upstream valve had been isolated. While loosening the 4-inch Victaulic fitting, the pipe separated due to tension within the line, causing one of the pipes to swing toward the employee, striking EE in the right lower chest.

Ee reports pulling on diesel hose after fueling up equipment, feeling sharp, twisting pain in abdomen. Finished fueling, after about 20 minutes they threw up. Continued to fuel island, parked, went to blue room, threw up. Reported to lead, transported to ER, diagnosed with hernia, referred for surgery. Working restricted.

EE reports loading truck w/mud, making 2nd pass, hit return to dig button, lowered bucket halfway. Bucket stopped instantly, jarring them in the cab. They reported the incident 4 days later on 1/19, and sought treatment. Diagnosed with lumbar sprain, released with restrictions site cannot provide modified duty under.

EE was walking across the parking lot to the autoclave when they slipped on snow covered ice, falling and landing on right shoulder. Their shoulder continued to hurt, and on 2/25 they were diagnosed with a soft-tissue tear, making this reportable for treatment. The EE resigned and left NGM, their last day was 2/26.

IP was removing bolts from the cutting edge of a loader bucket with an impact gun. The bolt came unseated, and a sharp edge on the bolt lacerated the IP's leg above the knee.

Bolter operator and mechanic were aligning carousel on bolter. Ee using a short scaling bar to rotate carousel for alignment. While pulling back, the bar slipped off the carousel collar, causing ee to stumble backwards and fall, hitting EE's head on bolt rack on front of bolter. Seen at ER, lac closed with staples, released with restrictions.

EE was changing out water pump when water from the pump splashed them in the face. They continued to work, finishing out the shift. The following day, they went to the clinic, was diagnosed with bilateral conjunctivitis. Released to return to work full duty.
