Employee was replacing a 120 pound grease drum on fuel truck. Employee decide to load grease drum by self without help and use of equipment resulting in strained lower back. Safety Manual states "Learn to lift the correct way, bend knees, keep back erect, and get help for heavy loads". Job description states "Able to lift up to 70 pounds".

Showing all 4While changing the spray bar pump, they removed the first pump and prepped to install the replacement pump. The pump was on forks of the 908 FEL, positioning to slide the pump from the forks to the pump stand. When the pump was slid off the forks, 1 ft. away, EE's knee buckled to the left breaking a bone in EE's leg.
Employee was relocating dredge and got stung by 20 - 30 bees resulting in Anaphylaxis.
Employee was pulling on a mechanism when EE felt a pop in EE's lower back.
