Showing all 26A maintenance crew was working on separator rotor cage. While lifting it with steel chokers, one of the steel chokers failed. Consequently, the other choker broke causing the cage to drop approximately 3 feet. As the cage fell, a maintenance repair got EE's fingers between the cage and the fan shroud where EE's hand was sitting, causing severe lacerations to 3 fingers
While loading coal on the pile, the operator backed down ramp and turn too soon causing loader to lay over on its side. Employee refused treatment initially, but was taken for evaluation the next shift. Operator was diagnosed with a concussion and given medication.
Employee was washing haul truck when lost balance and struck right side of chest on scaffold planking fracturing two ribs and injuring knee.
While working on the clinker transport system, a maintenance mechanic began to feel ill. Reported condition to supervisor. taken to ER and diagnosed with heat exhaustion.
While picking up used dust collector bags off of the ground, a maintenance mechanics stepped on a piece of clinker. This caused EE to lose EE's balance and twist EE's back while falling to the ground. On 7/1/25, medical evaluation was sought and the employee was placed on work restrictions.
After clearing the clinker chute, the employee attempted to exit the area and struck EE's nose on a cross member of scaffold. This resulted in a nasal bone fracture.
While descending s set of steps, an employee lost balance stepping onto the landing and fell to the ground. Employee felt pain in right knee and was taken for evaluation. Diagnosed with knee strain, placed on restricted duty and PT was prescribed.
An employee was at desk working on a stainless-steel junction box. While attempting to move the box off the desk, the box slipped. When regaining control of the box, a sharp edge on the hinge of the box cut the employee's left middle finger. The employee was taken to the local occupational clinic to have the wound cleaned and 3 stitches were used to close the wound.
Employee noticed a discomfort in left eye when preparing to go home for the day. Employee flushed the eye and went home. The discomfort increased throughout the night. The next day a foreign object was removed by eye specialist.
Employee stated they were pushing on the handle of a come along when they felt a sharp pain in their right shoulder. Employee was placed on restricted duty and PT on 12/11/23.
Maintenance Mechanic injured left bicep while installing a guard on the 143 tail pulley. EE was placed on restricted duty.
While operating 40t CAT haul truck, employee states steering failed causing haul truck to strike clinker cooler building wall. Employee complained of neck and shoulder pain and diagnosed with sprains of the neck and shoulder. Employee was placed on restricted duty.

Employee was descending the steps of the 102 conveyor. When reaching the last step, the employee rolled EE's left ankle. X-rays were negative and employee placed on restricted duty.
After finishing grinding a gudgeon shaft, a maintenance mechanic accidently grazed the leg of another maintenance mechanic who they were working with. This resulted in 7 stitches to close wound on lower left leg. 4" portable hand grinder found to be in good condition and no defects.

Shipping employee was moving rail with a track mobile. While moving a rail car loaded with cement derailed and laid on it side. This caused the track mobile to lay over on its side also. The operator struck EE's head on the inside of the cab which caused a laceration above the employee's eye. 4 stitches required to close wound.
While installing liners in the coal mill, employee stepped down approximately 24" into lower housing and felt a pain in the left heel. Diagnosed with a heel contusion and placed on restricted duty.
An employee was troubling an issue with a screw conveyor. The employee removed the guard from the drive belts while another went to reset the breaker. The employee had EE's hands on the belts when other employee reset the breaker and the motor attempted to start. The employee at the motor hand was pulled between the belt and the sheave.
While attempting to cut a 36" conveyor belt with a razor knife, a maintenance repairman lacerated the back of left thumb. This resulted in stitches to close wound.
