Employee reported experiencing lower back pain prior to work on 12/17/25 The employee stated the discomfort was noticed prior to the shift beginning however the cause of the injury is unknown and has not determined to be work-related at this time The employee sought medical evaluation and the matter is currently under investigation by the Company and workers compensation carrier.

OPERATOR WAS RUNNING A 930 LOADER AND WORKING ON THE HAUL ROAD A ROCK ROLLED OFF THE BERM AND THE OPERATOR RAN OVER THE ROCK CAUSING EE TO BE BOUNCED UP IN THE AIR AND CAUSING PAIN IN EE'S LOWER BACK AND RADIATING DOWN EE'S LEFT LEG

Truck trailer turned over while dumping.

Operator was climbing up the ladder on 777B rock truck, stopped on the bumper, fell to the ground landing on left side. Operator complained of pain in left shoulder, left hip, and lower back. Was taken to a hospital for X-Ray & CT scan, both came back ok with no breaks or fractures.

While raising/lowering a D10R belly pan, the belly pan got onto EE's chest.

While using a grinder to remove a rusty bolt the grinder kicked back and struck the chin.

Bottom step broke letting injured worker fall to the ground

While changing a hydraulic hose, EE stated EE felt bad. A co-worker evaluated EE and suspected dehydration. EE agreed to go get checked out at the local ER. The ER stated EE appeared to be dehydrated. The ER administered fluids and released EE to return to work.

Injured worker was removing a belly pan from a CAT 992D loader. The Pan fell approximately 3 feet and caught EE's right hand between the pan and an impact wrench.

Dozer was being worked on the dozer push arm slipped off roller frame and hit injured worker's lower leg and foot.
